ObjectiveThis study analyzes rabbit knee osteoarthritis model’s serum superoxide dismutase(Superoxide dismutase,SOD),malondialdehyde(Malondialdehyde,MDA),serum and synovial fluid interleukin-1 beta(Interleukin-1 beta,IL 1 beta)effects with patellar inward pushing,and to explore the mechanism of patellar inward pushing on the treatment of knee osteoarthritis,and provide reference and theoretical support for the clinical diagnosis and treatment of knee osteoarthritis.Method1.30 New Zealand white rabbits were divided into 5 groups by random number table method: blank group,model group,glucosamine hydrochloride group,patellar inward pushing group,and patellar inward pushing combined with glucosamine hydrochloride group,each group was 6.The experiment was carried out after a week of adaptation;2.Model group,glucosamine hydrochloride group,patellar inward pushing group and patellar inward pushing combined with glucosamine hydrochloride group,24 New Zealand white rabbits were built.The remaining 6 New Zealand white rabbits were the blank group,which did not do the modeling and other related treatment.After 1 weeks,the glucosamine hydrochloride group was treated with glucosamine hydrochloride,and the patellar inward pushing group was applied to the patellar inward pushing manipulation.The patellar inward pushing combined with glucosamine hydrochloride group was used for the treatment of patellar inward pushing manipulation combined with glucosamine hydrochloride gastric lavage.Push the patella operandi are special operation and manipulation treatment scheme: the rabbits were placed in lateral position fixed,to appease their emotions to be stable after the rabbit knee joint unbend,the operator palpable patella,with the thumb and index finger touch sub middle,margin of medial and lateral force balance,promote the rabbit patellar medial flat line,push to the limit position,hold for 5 seconds,then release 3 seconds to return the natural patella.The operation time of this method was 10 minutes,and 1 times a day for 8 weeks.Glucosamine hydrochloride treatment: animal stomach Glucosamine Hydrochloride Capsules,the dosage was 250mg/kg,dose volume was 5ml/kg,1 times / day,continuous treatment for 8 weeks.The two groups in the blank group and the model group did not do the related treatment;3.After modeling 9 weekends,with a protractor to measure the rabbit ipsilateral and contralateral knee extension and flexion angles,in order to calculate the joint activity;4.After modeling 9 weekends,take blood from the heart of 5ml,according to the operation steps of SOD test kit,xanthine oxidase method(hydroxylamine)for determination of serum SOD activity;in accordance with the steps of MDA test kit,the content of serum MDA determination;Determination of IL 1 beta content in serum and synovial fluid in use ELISA method;5.After completing heart blood collection and cytokine detection,all rabbits were sacrificed from air embolism of rabbit ear vein,and the cartilage of knee joint was collected.Gross and pathological observation was performed to evaluate the degeneration of knee joint cartilage and synovial membrane.Result1.Compared with the blank group,the activity of knee joint in model group decreased,the activity of SOD in serum decreased,the content of MDA increased,the content of IL-1 beta increased in serum and synovial fluid,and the observation score increased under general and light microscope(P<0.05);2.Compared with the model group,the knee joint activity of the patellar inward pushing combined with glucosamine hydrochloride group increased,glucosamine hydrochloride group,patellar inward pushing group and patellar inward pushing combined with glucosamine hydrochloride group,in these three groups the serum SOD activity were increased,the content of MDA decreased,the content of IL 1 beta in the serum and joint fluid decreased,and the scores of general and light microscopic observation and evaluation were reduced(P<0.05);3.And compared with the glucosamine hydrochloride group,there were no significant difference in the activity of knee joint,the serum SOD activity,the content of MDA,the content of IL 1 beta in the serum and the joint fluid,and the gross and light microscopic examination scores in the patellar inward pushing group(P>0.05).Compared with the group of glucosamine hydrochloride,the patellar inward pushing combined with glucosamine hydrochloride group’s serum MDA content decreased,the content of IL-1 beta in serum decreased,and the scores of general and light microscope decreased(P<0.05);4.No knee osteoarthritis was found in the knee joint of the blank group.There were different degrees of knee osteoarthritis in the model group,the glucosamine hydrochloride group,the patellar inward pushing group,the patellar inward pushing combined with glucosamine hydrochloride group.In the model group,the articular cartilage degeneration was the heaviest,and the pathological damage of the group of glucosamine hydrochloride and the patellar inward pushing group was light and similar,and the pathological injury of the patellar inward pushing combined with glucosamine hydrochloride was the lightest.Conclusion1.The decrease of serum SOD activity,the increase of MDA content and the increase of IL-1 beta in the serum and joint fluid of knee osteoarthritis suggest that SOD has protective effect on osteoarthritis,postpones articular cartilage degeneration,MDA and IL 1 beta are the inflammatory factors of osteoarthritis of the knee,aggravating the injury and degeneration of cartilage;2.Patellar inward pushing technique can significantly increase the SOD activity of serum in KOA animal model,reduce the content of serum MDA,reduce the content of IL-1 beta in serum and joint fluid,improve the ability to remove oxygen free radicals,reduce the release of inflammatory factors,promote the repair of articular cartilage and synovial membrane,and postpone the degeneration of the articular cartilage,which may be one of the effective mechanisms for patellar inward pushing on the treatment of KOA;3.Patellar inward pushing technique and Glucosamine Hydrochloride Capsules have good therapeutic effect on KOA,and there is no significant difference between the two treatments(P>0.05),but the combination of patellar inward pushing and Glucosamine Hydrochloride Capsules is better than the two alone.
