Objective By testing the blood glucose,insulin levels,adiponectin and insulin-like growth factor-1 and calculating the Body mass index of acne patients,finding the differences between patients with acne and the normal population,acne patients with different syndrome types,in order to further explore the relevant factors of acne incidence.Trying to provide possible objective indicators for acne syndrome differentiation.Methods A total of 44 acne Patients admitted to the dermatology acne outPatient clinic of the First HosPital of Wuhan City from October 2017 to February 2018 were included as a case group.Twenty healthy subjects were included as a control group.Divided the two groups into different TCM(traditional Chinese medicine)syndromes(Phlegm or no Phlegm).The body mass index(BMI)was calculated by measuring the height and weight of all the groups.Testing their blood glucose(OGTT test),insulin level(insulin release test),the serum level of insulin-like growth factor(IGF-1)and adipose Element(ADP).Applying Statistical software compare the BMI,blood glucose,insulin,IGF-1 and ADP between the two groups and different syndromes.Results1 Comparison between the experimental group and the control group: The fasting blood glucose level of the experimental group was significantly higher than that of the control group(P<0.05),including fasting,30 min after meal,60 min after meal,120 min after meal,and 180 min after meal.In the experimental group,the insulin level at 30 minutes after meal was significantly higher than that of the control group(P<0.05).The insulin at 180 minutes after meal was lower than that of the control group(P>0.05).There was no difference in insulin levels between the two groups(P>0.05).The mean values of BMI and IGF-1 in the experimental group were significantly higher than those in the control group(P>0.05).The mean ADP concentration in the experimental group was lower than that in the control group,with no statistical difference(P>0.05).2 Comparison of the experimental group of Phlegm and the experimental group of non-Phlegm : The fasting blood glucose level in the Phlegm group was higher than that of non(P<0.05),and there was no significant difference during the rest period(P>0.05).The insulin levels of the two groups was no difference(P>0.05).The mean BMI was higher and the ADP was lower in the Phlegm group than that of non,(P<0.05).The level of IGF-1 was no difference in the two groups(P>0.05).3 Comparison of BMI segments in the experimental group of Phlegm : In the experimental group,there was no significant difference in blood glucose and insulin between BMI<24 group and BMI?24 group(P>0.05).The ADP was significantly higher in the BMI<24 group than in the BMI?24 group(P<0.05).There was no significant difference in the IGF-1 between the two groups(P>0.05).4 Comparison of the experimental group of Phlegm with the control group: the blood glucose level in the experimental group of Phlegm was higher than that in the control(P<0.05);the fasting insulin in Phlegm group was higher than the control(P<0.05).There was no difference in insulin during the rest of the time(P>0.05).The BMI of Phlegm group was higher and the average ADP value was lower than that of the control.The mean IGF-1 was higher than that of the control group,and the difference was statistically significant(P<0.05).5 Comparison of the experimental group of non-phlegm with the control group : The blood glucose level of non-phlegm group was higher than that in the control at each time point(P<0.05).The insulin level had no difference in the two groups(P>0.05).There was no significant difference in BMI and ADP between the two group(P>0.05).The IGF-1 in the non-phlegm group was significantly higher than that in the control(P<0.05).6 Correlation analysis: There was a significant negative correlation between ADP and BMI(R=-0.496 or-0.529,both P<0.05).There was no significant correlation between ADP and HOMA-IR in the overall and experimental groups(P>0.05).There was no significant correlation between ADP and HOMA-IR in the experimental group(P>0.05).Conclusion The incidence of acne may be related to abnormal glucose metabolism and it is even more pronounced in acne of Phlegm.Over weight may be a risk factor for the development of acne,and IGF-1 is closely related to acne.Phlegm was closely related to BMI and ADP,and there was a significant negative correlation between ADP and BMI.The lower ADP was,the higher the BMI was,and there was no significant relationship between Phlegm and IGF-1.BMI and ADP may be considered as reference for acne of Phlegm classification.
