Research purpose:To explore the feasibility of using serum microRNAs as biomarkers for molecular screening of adenomyosis,the feasibility of serum microRNAs as biomarkers for differential diagnosis of adenomyosis and gynecological benign tumors,and the effect of Xuan Dan Sanjie Decoction on candidate microRNA of adenomyosis.Research methods:1.Experimental and clinical studies on serum microRNA as a screening index of adenomyosis.A total of 102 patients with adenomyosis diagnosed in Jiangsu Provincial Hospital of Integrated Traditional Chinese and Western Medicine from March 2015 to March 2018 were selected as the study group,at the same time,102 healthy examinees,whose age,sex and adenomyosis group were basically matched,were collected as the control group.All miRNAs in both serums were preliminarily screened by high-throughput sequencing technology,and then real-time fluorescence quantitative PCR(qRT-PCR)was used to screen and verify the initial miRNA.2.Serum microRNA as a differential diagnostic marker for adenomyosis and gynecological benign tumors.A total of 48 female patients(including adenomyosis,uterine fibroids,endometrial polyps,and endometriosis,12 cases each)were selected from March 2015 to March 2018 in Jiangsu Province Hospital of Integrated Traditional Chinese and Western Medicine,qRT-PCR was used to verify the expression of hsa-miR-22-3p,hsa-miR-103a-3p,hsa-miR-182-5p in the serum of the above 48 samples.3.Experimental study on serum microRNA of qi stagnation and blood stasis type adenomyosis with Xuan Dan Sanjie Decoction treatment.Twenty-four cases of adenomyosis diagnosed in Jiangsu Province Hospital of Integrated Traditional Chinese and Western Medicine from March 2015 to March 2018 were selected.Among them,12 cases were newly diagnosed patients without any treatment,and 12 cases who were diagnosed as qi stagnation and blood stasis type and were treated with Xuan Dan San Jie Decoction for 3 months.And then qRT-PCR was used to verify the expression of hsa-miR-22-3p,hsa-miR-103a-3p and hsa-miR-182-5p in the serum of the above 24 samples.Research results:1.Experimental and clinical studies on serum microRNA as a screening index of adenomyosis.The expression level of microRNAs in the serum of the study group was significantly different from that of the control group,with P<0.05.There were 316 up-regulated miRNAs and 24 down-regulated miRNAs.After further screening and verification,three serum miRNAs(hsa-miR-22-3p,hsa-miR-103a-3p,hsa-miR-182-5p)were found to have increased expression in patients with adenomyosis,and their AUC were respectively 0.900(95%CI,0.839-0.961),0.828(95%CI,0.747-0.909),0.844(95%CI,0.766-0.921).2.Serum microRNA as a differential diagnostic marker for adenomyosis and gynecological benign tumors.The expression of hsa-miR-22-3p and hsa-miR-182-5p in serum of adenomyosis patients was significantly higher than that of other three diseases,including uterine fibroids,endometriosis and endometrial polyps,P<0.05.The expression of hsa-miR-103-3p in adenomyosis was higher than that in endometriosis and endometrial polyps(P<0.05),but it was not significantly different from that in uterine fibroids.Difference(P>0.05).3.Experimental study on serum microRNA of qi stagnation and blood stasis type adenomyosis with Xuan Dan Sanjie Decoction treatment.There were significant differences in serum expression levels of hsa-miR-22-3p and hsa-miR-103-3p in patients with adenomyosis before and after treatment,P<0.05.However,there was no significant difference in the expression of hsa-miR-182-5p in serum of patients with adenomyosis before and after treatment,P>0.05.Conclusion:Hsa-miR-22-3p,hsa-miR-103a-3p,hsa-miR-182-5p can be used as molecular biological screening markers for adenomyosis.The above three miRNAs can be used in combination as a biomarker for differential diagnosis of adenomyosis and uterine fibroids,endometrial polyps,and endometriosis.Xuan Dan Sanjie Decoction can affect the expression of candidate hsa-miR-22-3p and hsa-miR-103-3p in adenomyosis,and these two miRNAs are expected to be efficacy observation indicators for qi stagnation and blood stasis type of adenomyosis with Xuan Dan Sanjie Decoction treatment.
