Short-term Prognosis In Patients With Unconsciousness Predicted By Mismatch Negative

Background and ObjectiveThere are many patients with consciousness disorder caused by TBI,cerebral hemorrhage,ischemic anoxia and other causes of brain injury in clinic,can they wake up? How long will it be possible to wake up? It has always been a problem for the sleepy clinician.To study prognosticate value of amplitude and latency of mismatch negativity in patients with conscious disturbance,through recording dynamically mismatch negativity.Method36 patients divided into research group,and 4 normal people divided into control group.In the day of admission,the mismatch negative wave was inspected and evaluated records of the patient's level of consciousness,every 2 weeks done this once,until the course of more than 3 months.The t test was used to analyze the difference of amplitude and latency of the mismatched negative wave between study group and the normal control group,and the linear regression analysis was used to analyze the relationship between the latency and amplitude of the first mismatched negative wave and the 3 months' consciousness level.ResultComparing amplitude of mismatch negative wave between normal control group and the study group,the difference was statistically significant[(3.63±0.33uV)vs(1.28±1.32uV)t=6.64,P<0.05)];comparing the latency,the difference was statistically significant [(217±40ms)(170±22ms)t=2.25,P<0.05 ].The relationship between amplitude of the first mismatch negative wave and the patient's level of consciousness was6 no statistical significance(r=0.34,P>0.05),but the relationship was statistical significance after 3 months.Compared latency with same method the difference was not statistically significant(u=0.29,p>0.05).when the negative wave amplitude reached more than 1.3uV for the first time,it can forecast whether patients can regain consciousness after 3 months,with sensitivity and specificity of 80% and 84%..Conclusionthe mismatch negative wave amplitude was lower and the latency wsa delayed in patient compared to normal people.amplitude of the mismatch negative recovery can precede the level of consciousness recovery,whcih can predict the prognosis of patients with coma,but latency has no predictive value.when the mismatch negative wave amplitude more than 1.3uV,consciousness recovery is more likely in short term(3 months).
