Backgroundcarcinoma(esophageal)is one of the most common malignant tumor in clinic [1].Esophageal carcinoma in the Middle East,Africa and Asia and parts of Europe has high incidence rate,which Chinese north and South Africa Turkey and Iran had the highest rate,the probability of African American patients of esophageal squamous cell carcinoma is about 5 times that of whites,blacks accounted for about 83%[2 of esophageal carcinoma.The pathogeny and mechanism of esophageal cancer is unclear,but it has a large number of researchers believe that differences in [3-4] occurrence and population characteristics of esophageal cancer,and cell levels were related to occurrence of esophageal cancer in China in addition to similar risk factors,race,socioeconomic status and diet and Gastric Cardia Achalasia,esophageal stenosis and the system sclerosis risk factors such as nitrate,the daily dietary intake of food,lack of zinc,selenium,vitamin C/,vitamin A,and excessive heat were also affected the occurrence and development of esophageal carcinoma [5].In recent years,the incidence of esophageal adenocarcinoma in Europe and the United States has exceeded the trend of esophageal squamous cell carcinoma,but squamous cell carcinoma is still the main [6] in our country.Although the medical development has produced a variety of treatment methods,including traditional surgery,radiotherapy and chemotherapy,and state-of-the-art molecular targeted therapy and immunotherapy,survival period has been significantly improved,but the 5 year survival rate of patients with esophageal cancer has overall is still low,the reason the main consideration is not clinical effective early diagnosis,and treatment,to prevent the recurrence and metastasis and poor [7].With the development of medical technology at home and abroad,molecular targeted therapy in recent years has provided new ideas for the clinical treatment of cancer,and has achieved good results.In a large number of studies,[8-9] also found that many biological targets not only participate in the normal physiological process of human body,but also closely related to the occurrence and development of malignant tumor diseases.And there are many scholars confirmed that [10-11] targeted drugs used in patients with non-small cell lung cancer has very good therapeutic effect,and clinical treatment of gefitinib,erlotinib and other targeted drugs have been widely used in patients with lung cancer,also let us see the bright prospect in the field of lung cancer treatment.But through the clinical study found that application of targeted therapy in the treatment of patients with esophageal cancer is still less,so to find a more accurate and effective molecular markers for early detection of malignant tumors,and even act as effective target for treatment of esophageal malignant tumor become particularly important.Tumor is formed by abnormal gene coding,and its development is closely related to inactivation of tumor suppressor gene,activation of proto oncogene and changes of DNA damage and related microenvironment.[12].And the evil of tumor progression is a complex and tedious steps of a process,including tumor cell adhesion,matrix degradation,along with blood metastasis process,the degradation of tumor stroma is the key step for the process of [13].The majority of cancer patients do not have typical symptoms in the early stage,and all kinds of malignant tumors have the tendency of early rising to distant metastasis,so the prognosis of most malignant tumor patients is poor.Therefore,the exploration of the mechanism of tumor cell development and the regulation factors associated with this process has become the focus and focus of the current industry scholars [14].MMP-10 and Cav-1,as an important indicator of the malignant progression of tumors,have been receiving more and more attention and recognition by foreign scholars [15].Previous studies have found that [16],matrix metalloproteinase-10(Matrix metalloproteinase-10 MMP-10)expression level in esophageal cancer and other malignant tumors significantly than in adjacent tissue,some scholars [17] esophageal cancer patients and healthy people in vitro mucosal tissue MMP-10 gene expression,results showed that the expression of MMP-10 in esophageal cancer green the mucosa was significantly higher than that in healthy control group.Chen Xiaochang and other [18] patients have also confirmed that the high levels of caveolin-1-1(caveolin-1,Cav-1)in serum of patients with esophageal cancer can be used as markers for esophageal cancer.Similar studies have been done in recent years,and recent studies have found that both may be related to tumor invasion,angiogenesis,metastasis and recurrence.ObjectiveTo investigate the expression of matrix metalloproteinase-10(Matrix metalloproteinase-10 MMP-10),-1(caveolin-1,Cav-1 caveolin)expression in the serum of patients with esophageal carcinoma,and analyze the correlation between the level and clinical prognosis in patients with the expression of the two,in order to provide reference for the diagnosis and evaluation of esophageal cancer patients pre.Methods in The first people's hospital of Zhumadian from January 2012 to April 2014 admitted patients with esophageal cancer and 68 cases as the observation group,all patients underwent surgical treatment to pathological diagnosis of esophageal cancer and staging;and 20 cases were selected in the hospital examination of normal esophageal mucosa in control group.The expression of SP was detected by immunohistochemistry MMP-10 protein,using Western blotting method to detect Cav-1,compared to observe the relationship between the group Cav-1 and MMP-10 positive expression and clinical pathological analysis of Cav-1,and the relationship between MMP-10 expression and clinicopathological features of esophageal cancer patients,to analyze the effect of Cav-1,MMP-10 gene expression and factors.According to gender,age,tumor diameter,infiltration,TNM stage,degree of differentiation and the occurrence of lymph node metastasis group,comparison of different Cav-1 and esophageal cancer clinical and pathological features of patients with MMP-10 positive expression rate,and there was a statistically significant difference between the above factors as independent variables were Cav-1,MMP-10 positive expression of influencing factors Logistic regression analysis.In addition,the correlation between the positive expression of Cav-1 and MMP-10 and the correlation between the expression of Cav-1 and MMP-10 and the prognosis of patients with esophageal carcinoma were analyzed.Results1.the positive rates of MMP-10 and Cav-1 in the observation group were 85.29% and 77.94% respectively,while those in the control group were 25% and 15%,respectively.The difference between the two groups was significant(chi 2,27.624,26.458,P,respectively,<0.01).The observation group MMP-10 mRNA and Cav-1 mRNA expression respectively(2.73 + 1.05),(3.69 + 1.32),were significantly higher than those in control group(0.49 + 0.09),(1.02 + 0.56),the difference between groups was statistical,t = 9.492,8.788,P <0.01.Relationship between the expression of 2.MMP-10 and Cav-1 and clinicopathological features of esophageal carcinoma:The positive expression rates of MMP-10 were significantly different in different degree of infiltration,TNM stage,Z values were 2.620,3.467,P = 0.022,0.001;and with / without lymph node metastasis in patients with MMP-10 positive expression rate statistics,X 2=25.312,P<0.01.The positive expression rates of Cav-1 were significantly different in different degree of infiltration,TNM stage,Z values were 2.486,4.186,P <0.05;and with / without lymph node metastasis in patients with Cav-1 positive expression rate statistics,X 2=24.993,P<0.01.3.after factor Logistic regression analysis,tumor invasion,esophageal adventitia,III+IV stage and lymph node metastasis were the influencing factors of positive expression of MMP-10 and Cav-1 in esophageal cancer patients,and P was <0.05.4.by Pearson correlation analysis,there is a positive correlation between the expression of MMP-10 and the expression of Cav-1 in cancer tissues of patients with esophageal cancer,r=0.594,P<0.05.5.there were significant differences in the total effective rate of esophageal cancer patients with different TNM stages(P<0.05),and the total effective rate of stage I,II and III esophageal cancer was significantly higher than that of IV.In the early stage,16 cases showed grade I nausea and vomiting,and 9 cases showed a decrease in white blood count,all of which were mild.6.66 cases were followed up,2 cases were lost,the follow-up rate was 97.06%,and the median follow-up time was 26 months.The follow-up rates of 1 years,2 years and 3 years were 89.39%,61.76% and 41.17% respectively.With the prolongation of follow-up time,the positive rate of MMP-10 and Cav-1 showed a downward trend,but there was no statistical difference(P>0.05).7.the risk analysis model of Cox ratio,whether the survival time of patients with esophageal cancer lymph node metastasis,the positive expression of Cav-1 and MMP-10,so the factors as independent predictors of prognosis of patients with esophageal cancer.Conclusion1.the positive rates of MMP-10 and Cav-1 in esophageal cancer tissues were significantly higher than those in normal tissues,and the expressions of MMP-10,mRNA,Cav-1 and mRNA were also significantly higher than those in normal test tube tissues.2.esophageal invasion,TNM,II+III and lymph node metastasis are the risk factors for the positive expression of MMP-10 and Cav-1 in esophageal carcinoma tissues.3.the positive expression of MMP-10 and Cav-1 in cancer tissues of two esophageal cancer patients is positively correlated.4.with the gradual extension of follow-up time,the positive expression rate of MMP-10 and Cav-1 showed a downward trend,but there was no statistical difference.
