Study On The ED50 And ED95 Of Cis Atracuriun In The Removal Of Foreign Body In Asprive In Children

Objective To study patients with airway foreign bodies in the operation which can interrupt the research of glottic closure reflex,of cisatracurium in the ED50 and ED95.To provide a theoretical basis for the improvement of anesthesia scheme of airway foreign body surgery,and to provide a basis for improving the safety of anesthesia in critically ill surgical airway foreign bodies.Method Collected a total of 2 years old children who were with airway foreign bodies and were admitted to the operation room of Hunan Children's Hospital from May 2016 to October 2016.A total of 28 children were included in the study,and the systemic ASA score was II~III.All the children with foreign body removal in intravenous anesthesia.According to the order of children before and after surgery using up-and-down method given different doses of cisatracurium for surgery.If a patient who had positive reaction,cisatracurium blood concentration of a children to a lower dose(step the adjacent concentration difference was 0.05mg/kg).If the negative reaction whichare the glottal reflex is not interrupted.If the suspicious reaction,next patient will receive a repeat dose.The basic data and clinical observation indexes were recorded.Result 1.This research selected the 28 cases children,there were22 males and 6 females,with age(17.210 ± 3.785)months,weight(10.450± 1.233)kg,the recovery time(5.860 ± 1.957)min,the operation time(10.710 ± 3.700)min.Foreign body types are most common in peanut.The location of foreign body appears at the right and left sides,and the bronchial opportunity is equal.The vocal cords were open and free of motion in all patients.2.In this research program under anesthesia,cisatracurium in children with vocal cord closure reflex which ED50 interrupt for 0.066mg/kg,CI for 95% confidence interval was(0.061,0.071)mg/kg;ED95 was 0.078 mg/kg,CI for its 95% confidence interval was(0.072,0.124)mg/kg.3.There was no significant difference in HR,RR,Sp O2 and MBP in 28 children in group T0,T1 and T2(P>0.05).4.Of the children suspected of repeated administration,8 of them developed spontaneous breathing movements without coughing,choking,breath holding,and airway spasm.Conclusion 1.Cisatracurium in anesthesia can be safely used for removal of bronchial foreign bodies in children.2.In this study,in the airway foreign body surgery was interrupted by interruption of the cisatracurium,and the ED50 of the closed loop reflex of the cisatracuriumwas 0.066 mg/kg,and the ED95 was 0.078 mg/kg.3.Seven-halothane synergy of cisatracurium with inhibition of glottal closure and interrupt reflex glottic closure reflex,can effectively prevent cough,breath and airway spasm,the dose time curve of relationship specific needs further study.4.There is a certain dose range of cisatracurium can inhibit glottal closure reflex,preventing cough,choking breath and airwy.
