| ObjectiveTo measure the mental status and needs of ICU coma patients’ family members,and to analyze the influencing factors.to explore the relationship among anxiety,depression,needs and psychological resilience.To provide the clinical references for psychological support of ICU coma patients’ families.MethodsThis is a mixed research,including two parts,the qualitative research and quantitative research.(1)In qualitative research section,The simple random sampling method was adopted and the deep interview was conducted among 25 ICU coma patients’ realtives in a three grade a hospital in Changsha to explore the experience and needs.(2)In the quantitative research,the simple random sampling method was used to select 210 ICU coma patients’ family members,who were asked to answer the questionnaires of General information questionnaire,HAD,CD-RISC and CCFNI in a Hospital of changsha from 2015 to 2016.Results(1)The results of qualitative interview: the psychological state related topic of ICU coma patients’ family members was:(1)shock and fear;(2)sadness and self-blame;(3)anxiety and irritability;(4)depression and entanglement;(5)optimism.Besides,the requirements related topic were:(1)knowing the illness progress;(2)visit with patients;(3)assurance of best care;(4)social and economic aid;(5)Self-comfort.(2)The results of quantitative research:(1)The depression score of ICU coma patients’ family members was(13.54±5.06),74.8% of whom with depression.The anxiety score of ICU coma patients’ family members was(14.75±5.27),and 77.1% of family members with anxiety.The anxiety and depression were affected by the the patient’s course,the family members’ age,the level of culture,the state of work,the relationship with the patient,and so on,but not the gender.The course of disease had a great influence on the depression,while the age of family had a greatest impact on family anxiety.(2)The psychological needs of ICU coma patients’ family members were mainly lay in the comfort factor(2.32±0.51)and their support factor(1.69±0.28).Other factors from high to low were: close-to-patient factor,access to information factor,disease assurance factor.The needs of family members were affected by duration of disease,coma time,medical expenses,family members’ age,educational level,working status.The families of coma patients with higher education were more likely to receive information than the families in junior high school.In-service patients’ families had a stronger demand of receive support and comfortable than the retirement groups.The patients’ families who paid medical expenses by their own had a stronger demand of support than other groups.Among them,the source of medical expenses had the greatest impact on the psychological needs.(3)Psychological needs was significantly correlated to the psychological resilience.There was a positive correlation between the tenacity factor and access to information needs of family members(r=0.149,P=0.031).The Power trait were significantly negatively correlated with the need of perceived support,comfort,access to the patient,and condition guarantee.Optimism trait was positively correlated with the need of perceived support,comfort,access to information,and proximity to patients.(4)Psychological needs was related to the anxiety and depression,while the anxiety and depression were positively correlated to the needs of support,comfortable,close to the patients,and access to information(P<0.05).(5)The anxiety and depression was related to the psychological resilience.Tenacity and strength characteristics were negatively correlated to the anxiety and depression(P<0.01).Conclusions(1)ICU coma patients’ family members generally experienced anxiety,depression and other negative emotions,however,small number of positive and optimistic attitude remains.The anxiety and depression were affected by the the patient’s course,the family members’ age,the level of culture,the state of work,the relationship with the patient,and so on,but not gender.(2)The psychological needs of ICU coma family patients from high to low were: the comfort factor,support factor,close-to-patient factor,access to information factors,disease assurance factor.The needs of family members were affected by the duration of disease,coma time,medical expenses,family members’ age,educational level,working status.(3)Mental state,psychological needs,psychological resilience influence each other. |
