Objective: Toobserveparecoxib sodium associated with hydromorphone for treatment efficacy and safety of hyperalgesia after remifentanil anesthesia.Methods: 60 patients who undergoed elective thyroid surgery,were randomly classified into parecoxib sodium group(group P),hydromorphone group(group Q),and parecoxib sodium associated withhydromorphone group(L group).There were 20 patients in each group.Patients were administrated with routine monitoring,established vein channel,andweretreated with total intravenous anesthesia for continuous infusion of propofol 4-12 mg/kg.h and remifentanil 0.15-0.3?g/kg.min.Patients who were belonging to group Pdid intravenous injectionof 40 mg parecoxib sodium20 min before the operation.Patients ofgroup Q were injected with 10 ug/kg hydromorphone 15 min before the end of the operation.Group L did intravenous injection of 40 mgparecoxib sodium 20 min before surgery and did intravenous injection of 10 ?g/kg hydromorphone 15 min before the end of surgery.Observation index: Record age,gender,weight,operation time,dosage of remifentanil and propofol;recovery time and extubation time;wake up time,before extubation and after extubation Sedation agitation Scale;after immediatelyextubation(T0),15 min after extubation(T1),30 min after extubation(T2),2h after extubation(T3),4 h after extubation(T4)Numerical rating scaleand Ramsay sedation score;and within 24 hours after the adverse reactions(such as nausea and vomiting,respiratory depression,skin itching and other cases).Results: Three groups of patients with general information,operation time,propofol and remifentanil dosage,recovery time and extubation time,SAS score,Ramsay sedation score and the incidence of adverse reactions are not statistically significant(P>0.05).Compare the three groups of patients with postoperative Numerical rating scale,group Lare lower than those ingroup P and group Q at each time point,group Pare lower than in group Q at each time point,the differences are statistically significant(P<0.05);the comparison group,P,Q and L groups are T0 > T3 > T4(P<0.05).Conclusion: 1.Doing intravenous injection 40 mg parecoxib sodium 20 min before surgery to prevent hyperalgesia after remifentanil anesthesia effect is better than intravenous injection of 10 ?g/kg hydromorphone15 min before surgery.2.Doing intravenous injection 40 mgparecoxib sodium 20 min before surgeryassociated with postoperative intravenous injection of 10 ?g/kg hydromorphone 15 min before the end of surgery is better than the single drug.
