Objective: To investigate the protective of unfractionated heparin on pulmonary vascular endothelial glycocalyx in septic rats.Methods:1.30 male SD rats(weighing 220g-260g)were divided into 3 groups randomly: Sham control group(Group SC,n=10);Sepsis gro u p(G r o u p S E P,n = 1 0)a n d H e p a r i n g r o u p(G r o u p H E P,n = 1 0).2.Three groups of rats were in abrosia 12 h before molding.The cecal ligation and puncture(CLP)was utilized in Group SEP and Group HEP and the sepsis rat were modeled.Only laparotomy and no CLP was performed in Group SC.3.Group HEP was injected unfractionated heparin for 100U/kg via abdominal cavity one hour after operation.4.Supervising the activities、psychosis、diet、water and breathing of rats after operation.At each time of 4h,8h and 12 h after operation,starting blood sampling via vein in rats and using ELISA to detect inflammatory mediators TNF-ɑ,glycocalyx components Syndecan-1、Heparan sulfate and Heparanase;The rats after 12 h the blood sampling to obtain the specimen of lung tissues,the left lung ti sse calcute the psychrometric ratio,the right lung tisse observe the structure of pulmonary vascular endothelial glycocalyx through transmission electron mirror.Results:1.The water content of pulmonary tisse:Group SEP and HEP is higher than Group SC(P<0.05),Group HEP is lower than Group SEP.2.Pulmonary vascular endothelial glycocalyx :Group SC:the glycocalyx was continuous and was distributed evenly.The structure of the glycocalyx was very compact.Group SEP:the glycocalyx was not continuous and lots of disruption was appeared.The distribution of glycocalyx was unevenly and the structure was obviously porous.Group HEP:In this group,glycocalyx was not continuous and had disruption in it as well as unevenly distribution.The structure ofthe glycocalyx was porous,but the degree of disruption and porosity was lighter than that in Group SEP.3.Using ELISA to detect the level of the serum inflammatory mediators TNF-ɑ:through the comparison between different groups at the same time,the statistical significance(P<0.05)in the compariso n of among three groups at 4h,8h and 12 h was found when every two of them was compared.Group SEP and HEP is higher than Group SC(P<0.05),Group HEP is lower than Group SEP;glycocalyx components Syndecan-1、Heparan sulfate:through the comparison between different groups at the same time,the statistical signifycance(P<0.05)in the comparison of among three groups at 4h,8h and 12 h was found when every two of them was compared Group SEP and HEP is higher than Group SC(P<0.05),Group HEP is lower than Group SEP;Heparanase:through the comparison between different groups at the same time,the statistical significance(P<0.05)in the comparison of among three groups at 4h,8h and12 h was found when every two of them was compared.Group SEP an d HEP is higher than Group SC(P<0.05),Group HEP is lower than Group SEP.Conclusion1.In the model of septic rats,visible damages appeared in the pulmonary vascular endothelial glycocalyx and pulmonary edama.2.Unfractionated heparin can obviously have protective effect on Pulmonary endothelial glycocalyx and pulmonary edama in septic rats and the mechanism is possibly connected with the functions that heparin reduces serum,anti-inflammatory action made by TNF-ɑ and also decreases the level of heparanase.
