Chronic obstructive pulmonary disease(Chronic obstructive pulmonary disease,COPD)is a kind of airflow limitation,with forced expiratory maximal expiratory flow decreased as the main feature of the disease to slow the progression of age-related chronic respiratory disease.At the same time,the chronic inflammation of the airway,lung parenchyma and pulmonary blood vessels is the basic disease,and most of the airflow obstruction is irreversible,but sometimes it can show different degrees of reversibility and airway hyperresponsiveness.COPD is often associated with pulmonary heart disease,and even right heart failure,it is difficult to early diagnosis.Amino terminal pro brain natriuretic peptide(N-amino terminal Natriuretic Peptide,NT-proBNP),also known as the N terminal B type natriuretic peptide,was used in the diagnosis of coronary heart disease and left heart failure in the past.The clinical value of NT-proBNP in the diagnosis of COPD complicated with pulmonary heart disease,especially right heart failure.Objective:Objective to study the correlation between plasma NT-proBNP level and NT-proBNP and other clinical indexes in patients with COPD and COPD complicated with cor pulmonale.Materials and methods:This study was a retrospective,controlled study.A total of 145 patients with COPD and healthy physical examination in our hospital from September 2015 to September 2016 were selected as the subjects.All subjects were divided into five groups,control group A and study group B,C,D,E: A group of healthy subjects,a total of 30 people,the average age of 60.3 years;group B stable COPD group,a total of 23 cases,the average age of 64.7years;C group COPD acute exacerbation group,a total of 29 cases,the average age of 70.1years;group D with chronic pulmonary heart disease COPD,lung and heart function decompensated group,a total of 31 cases,the average age of 74.7 years;group E withchronic pulmonary heart disease COPD,lung and heart function decompensated group,a total of 32 cases,the average age of 76.6 years.E group of 32 cases according to the American Heart Association of New York(NYHA)grading standards were divided into grade II group of 10 cases,grade III group of patients in the group,the fourth grade group of 8 cases.Results:The levels of NT-proBNP in the B,C,D,E four groups were significantly higher than those in the control group,A group,and gradually increased with the severity of the disease.The plasma NT-proBNP level of D and E two groups was significantly higher than that of COPD group and C group,respectively,with COPD in combination with B.B,C,D,E four groups of NT-proBNP level and right ventricular diastolic diameter(RV),right ventricular outflow tract width(RVOT),pulmonary artery pressure(PASP)levels were positively correlated with FEV1(FEV1%)was negatively related to the level of.Conclusion:The level of plasma NT-proBNP in patients with COPD complicated with pulmonary heart disease was significantly higher than that in patients with simple COPD,and the higher the severity of heart failure,the higher the level of NT-proBNP.In addition,the level of NT-proBNP was correlated with RV,RVOT,PASP,FEV1% and other clinical indicators.Plasma NT-proBNP level has a good clinical value in the diagnosis and evaluation of COPD with pulmonary heart disease.
