Comparsionof The Curative Effect On Treatment Of Intertroichanteric Fractures With Intertan And PFNA-?

Objective: Comparsion of the curative effect on treatment of closed reduction and fixation intertrochanteric fractures with InterTan and PFNA-?.Methods: Between February 2014 and April 2015,86 patients of femoral intertrochanteric fractures were retrospectively reviewed.the patients were divided into two groups:40 cases with the fixation of InterTan(20 male,20female),their age was71.25±13.39 years,according to the Evans-Jensen classification:2 cases of type ?,5 cases of type ?,14 cases of type IV,19 cases of type ?;46 cases with the fixation of PFNA-?(22 male,24female),their age was 72.02±11.09 years,according to the Evans-Jensen classification:4 cases of type?,9 cases of type ?,16 cases of type IV,17 cases of type ?.We assessed the clinical results(operation time,intraoperative blood loss)and postoperative radiographic results of the two groups at the early term;We assessed the results of fracture healing time and patients were clinically evaluated using the Harris Hip Score at the intermediate term.Results:Two groups of patients were all followed up for 10-24 months.Fractures in patients were all healed.No complications(fracture,infection,DVT,failure of internal fixation,fracture displacement,avascular necrosis,femoral neck cut-out,hip varus deformity,et al.)were occurred.Clinical results: Operation time was(87.03±23.34)min in the Inter Tan group,(60.39±12.59)min in the PFNA-?group,these differences were statistically significant(P<0.05).Intraoperative blood loss was(203.50±88.22)ml in the Inter Tan group,(158.15±79.96)ml in the PFNA-?group,these differences were statistically significant(P<0.05).Harris Hip Score(6 months after surgery)was(83.93±13.50)points in the Inter Tan group,(85.17±11.89)points in the PFNA-?group,There was no statisticallysignificant difference between the two groups(P>0.05).Radiographic results: Separation distance of the fracture:In the group A(Separation distance 0mm),there were 32 cases in the Inter Tan group,27 cases in the PFNA-?group;In the groupB(Separation distance 0-2mm),there were 8 cases in the Inter Tan group,16 cases in the PFNA-?group;In the group C(Separation distance >2-3mm),there were 0case in the Inter Tan group,3 cases in the PFNA-?group,these differences were statistically significant(P=0.05).Fracture healing time was(12.00±3.39)weeks in the Inter Tan group,(13.65±3.73)weeks in the PFNA-?group,these differences were statistically significant(P<0.05).Conclusion:Inter Tan and PFNA-II are both the ideal fixations for intertrochanteric fractures.Inter Tan had stronger compression on fractures,fracture healing time was shorter.Operation of PFNA-II was easier,operation time was shorter,intraoperative blood loss was less.
