| Objective:To analyze the factors affecting the diagnostic yield of endobronchial ultrasound-guided transbronchial needle aspiration(EBUS-TBNA),and to provide a clinical basis for improving the diagnosis of EBUS-TBNA.Methods:There are 612 patients who selected from January 2013 to March 2016 in the First Affiliated Hospital of Soochow University.The effects of each factor on the diagnosis yield of EBUS-TBNA were analyzed from age,sex,smoking,anesthesia,the way of admission,number of lymph nodes undergoing biopsy,size of the largest lymph node,the location of lymph nodes and PETCT scan.Results:Among 612 patients,436 patients obtained positive results by EBUS-TBNA,the positive diagnosis rate was 436/612(71.2%).There was no significant difference between age(OR,0.868;95%CI,0.581-1.297;P=0.490),anesthesia(OR,1.221;95%CI,0.444-3.354;P=0.699),the way of admission(OR,0.960;95%CI,0.658-1.400;P=0.830,and the location of lymph nodes(?~2=7.828,P=0.166).Diagnosis yield was associated with sex(OR,11.041;95%CI,4.229-28.825;P<0.001),smoking(OR,12.355;95%CI,4.840-31.541;P<0.001),biospy of more than two sites(OR,1.927;95%CI,1.288-2.883;P<0.001),size of the largest lymph node(?1cm vs 1-2cm,OR,3.340;95%CI,2.020-5.521;P<0.001;1-2 cm vs>2cm;OR,1.692;95%CI,1.087-2.633;P=0.020),and positive PETCT scan(?~2=72.000,P<0.001).Conclusions:1.There is high diagnostic yield for EBUS-TBNA to definite the properties ofmediastinal lymph node,and there is an important meaning for us to diagnose mediastinal disease.2.Diagnosis yield can be affacted by sex,smoking,number of lymph nodes undergoing biopsy,size of the largest lymph node and PETCT scan.And There was no significant difference between age,anesthesia,the way of admission and the location of lymph nodes.3.Based on the research,in order to improve the diagnostic yield of EBUS-TBNA,we suggest choosing the largest lymph node,more lymph nodes undergoing biopsy and PETCT to obtain higher diagnosis yield. |
