Objectives:The aim was to checkthe level of serum heme oxygenase-1?HO-1?and the score of Montreal cognitive assessment?MoCA?of patients with severe obstructive sleep apnea hypopnea syndrome?OSAHS?before and after treatment,and to analyze the correlation among serum HO-1 level,MoCA,LSaO2 anf AHI,and to observe the effect of CPAP combined with surgical treatment on improving cognitive function in patients with OSAHS,which helpedto explore the predictive value of HO-1 as the role of cognitive function and the possible biochemical mechanisms of cognitive decline in patients with OSAHS.Method:80 patients were selected as samples who were diagnosed severe OSAHS through PSG as experimental subject in the period from October2015 to October 2016,After hospitalization,all patients were accepted comprehensive treatment?CPAP therapy plus 5 days of surgery treatment?.then it investigated the venous blood at the next morning and comprehensive treatment after 3 months,respectively,and the MoCA score was evaluated at the same time?15 cases were losted at 3 months during follow-up observation?.The change of serum HO-1 levels,Mo CA score,LSaO2and AHI before and after treatment was compared.The correlation of serum HO-1 levelswith each index was analyzed.The level of serum HO-1were measured by enzyme-linked immunosorbent assay?ELISA?.All the experimental data were analyzed by SPSS21.0 statistical software.Results:1.The level of serum HO-1 of the patients with severe OSAHS before andafter comprehensive treatment was8.30±0.33ng/ml?6.10±0.37ng/ml,the difference have statistically significant?P<0.05?.2.The MoCA score of the patients with severe OSAHS before and after comprehensive treatment was24.38±1.05 score?26.04±1.09 score,the difference have statistically significant?P<0.05?.3.The AHI of the patients with severe OSAHS before and after comprehensive treatment was 51.18±6.03times per hour?25.62±6.63times per hour,and the LSaO2 was 71.60±6.17%?80.40±4.39%,the difference have statistically significant?P<0.05?.4.Using Partial correlation coefficient,there was a negative correlation between HO-1 level and MoCA scorein severe OSAHS patients?r=-0.689,P<0.01?,and negatively correlated with LSaO2?r=-0.692,P<0.01?);there was a positive correlation between HO-1 level and AHI?r=0.695,P<0.01?;Mo CA score was positively correlated with LSaO2?r=0.634,P<0.01?,and showed a negative correlation with AHI?r=-0.614,P<0.01?.Conclusion:1.The level of serum HO-1 in patients with severe OSAHS was significantly increased,and HO-1 level was negatively correlated with the MoCA score,which suggested that HO-1 may be one of machanisms of pathophysiological process of cognitive decline.2.Comprehensive treatment which CPAP combined with surery was an effective measure to reduce the level of serum HO-1 and improve cognitive function in patients with severe OSAHS in the short term.
