| Objective: To evaluate the clinical efficacy of clavicular hook plate and Endobutton plates in the treatment of acute acromioclavicular joint dislocation,evaluate the effect of Endobutton plate in clinical treatment,and explore its advantages and feasibility.Methods: This study collected the clinical data of 70 patients who met the inclusion criteria from May 2012 to May 2016 in our hospital.Using the method of retrospective study,the subjects were divided into two groups according to different surgical methods: clavicular hook plate operation group and Endobutton plate operation group.In this study,we compared the average length of incision,operation time,intraoperative blood loss,hospital stay time,visual analog scale(VAS)score of the second day and one year after operation,shoulder constant score after one year operation,flexion on the move and outreach activities and postoperative complications.All the observed indicators were analyzed by SPSS 19.0 statistical software.Results: In the Endobutton plate operation group,the mean length of the incision was shorter than the clavicular hook plate operation group {(5.2±1.5)cm vs(6.6±1.4)cm},while the operation time was longer {(100.3±27.5)min vs(62.9±15.3)}(P<0.05).There was no significant difference in the amount of bleeding and hospital stay time between the two groups(P>0.05).Also,there was no significant difference in VAS score of the second day after the operation between the two groups(P>0.05),but the degree of pain in the VAS score group was lower after one year of the operation in the Endobutton plate operation group than that in the clavicular hook plate group{(1.2±0.6)vs(1.6±0.8)}(P<0.0001).There was significant difference between the two groups(P<0.0001)in the shoulder constant score after one year operation,especially in the pain,activity,daily activities and the total score parts{the total score:(86.0±6.6)vs(75.7±5.9)}.Meanwhile,in the clavicular hook plate group,the shoulder flexion and abduction on the activities was limited,(97.5±18.8)° and(96.5±10.3)°},respectively,which were significantly lower than those of Endobutton plate group {(142.3±9.4)° and(139.7±8.9)°}(P<0.0001).Postoperative complications of clavicular hook plate group was significantly more than Endobutton plate group.Conclusion: Treatment of acute acromioclavicular joint dislocation by Endbutton plate is more suitable for anatomical and biomechanical characteristics.It is a kind of elastic fixation with small trauma and quicker function of shoulder joint,with less complication and no need for secondary removal of internal fixation and other advantages.As a result,Endbutton plate is an ideal surgical method and in the clinical worthy of promotion. |
