Objective:To investigate the value of three-dimensional ultrasonography in the differential diagnosis of benign and malignant breast lesions.Materials and methods:One hundred patients with a total of 121 breast lesions were enrolled in our study.Each lesion was examined by two-dimensional and three-dimensional ultrasonography before the operation.According to the two-dimensional and three-dimensional ultrasonography of the lesion,the benign and malignant lesions of the breast were evaluated and compared with the postoperative pathological results.Logistic regression model was established to analyze the value of lesion morphology,border,microcalcification,posterior echo attenuation,anglur or burr,convergence sign,and blood flow classification in the differential diagnosis of its benign and malignant.The regression analysis was visualized by using R3.3.2 statistical software,and evaluating the value of three-dimensional ultrasonography in predicting the benign and malignant of breast lesions through receiver operating characteristic curve(ROC).If P <0.05,the difference was considered statistically significant.Results:(1)A total of 121 lesions,66 cases of benign lesions,55 cases of malignant lesions.The diagnostic coincidence rate of two-dimensional ultrasound was80.9%,the diagnostic coincidence rate of three-dimensional ultrasound was 93.3%.(2)The sensitivity of the two-dimensional ultrasonic diagnosis of breast lumps right was 87.88%,specificity of 89.09%,positive predictive value was 90.62%,negative predictive value of 85.96%.(3)The sensitivity,specificity,positive predictive value and negative predictive value of three-dimensional ultrasonography in diagnosis of breast lesions were 93.94%,92.73%,93.94% and 92.73% respectively.(4)there was no significant difference between the breast cancer and the results of pathological diagnosis by color Doppler classification(P > 0.05).(5)Logistic regression analysis showed that the morphological,border,microcalcification and angular or burr of breast lesions were statistical significant in two-dimensional and three-dimensional ultrasonography(P<0.05).The convergence sign of the three-dimensional ultrasonography on benign and malignant breast lesions was statistical significance(P<0.05).The posterior echo attenuation of benign and malignant lesions has no statistical significance(P>0.05).Multivariate analysis confirmed that only angular or burr,convergence sign and border were statistical significant in the diagnosis of breast cancer(P <0.05).The alignment chart graphically shows the percentage of these three ultrasound signs for estimating the risk of mass as a malignant.(6)ROC curve analysis showed that the area under the ROC curve of two-dimensional and three-dimensional ultrasound was 0.712 and 0.894 respectively.The diagnostic effect of three-dimensional ultrasound on breast lesion was better than that of two-dimensional ultrasound.Conclusion:(1)Two-dimensional ultrasound is shown in the tumor boundary,internal echo and blood flow could help breast lesions of benign and maligant.(2)Angulation or burr,convergence sign and boundary valuable to the diagnosis of breast malignant lesions.(3)Three-dimensional ultrasound on the differential diagnosis of breast mass is better than two-dimensional ultrasound.
