Purpose:To evaluate the effect of en bloc mesogastric excision in radical gastrectomy for gastric cancer.Method:Retrieved in February 2017 before the publication of the comparison of en bloc mesogastric excision and D2 radical gastrectomy in gastric cancer radical surgery in the literature.According to the inclusion criteria,the quality score was selected and the clinical effect index was extracted.The data of the included data were analyzed by Rev Man 5.3 software.Result:Finally,a total of 4 patients were included in the study,of which there were 331 patients in the en bloc mesogastric excision group,and in the D2 radical gastrectomy group(n = 167).With the D2 radical gastrectomy group,the en bloc mesogastric excision group operation time(MD=-14.52,95%CI=-23.64--5.40,P=0.002),bleeding(MD=-46.45,95%CI=-69.41--23.49,P<0.001),the difference was statistically significant postoperative exhaust time(MD=-0.54,95%CI=-0.87,--0.20,P=0.002)in lymph node the number of cleaning(MD= 2.92,95%CI=-1.66--7.51,P=0.21),postoperative hospitalization time(MD=-0.71,95%CI=-1.53-0.11,P=0.09),the incidence of postoperative complications(RR= 0.74,95%CI= 0.31-1.75,P=0.50)had no significant difference.Conclusion:the en bloc mesogastric excision group and D2 radical gastrectomy,shorter operation time,less intraoperative bleeding,earlier postoperative exhaust time,and the number of lymph nodes and D2 radical gastrectomy under the same premise,does not increase the incidence of hospitalization and patients with postoperative complications.With safety and feasibility,the en bloc mesogastric excision has the potential to become the standard operation for improving the prognosis of gastric cancer.
