| Objective:To investigate the clinical effect of endoscopic resection of pituitary adenoma by endoscopic resection of pituitary tumor and related complications.Method:253 cases from March 2012 to March 2016 of pituitary adenoma patients in neurosurgery department of Ji Ning NO.1Hospital were select,which were randomly divided into two groups:the endoscope group(observation group)and the microscope group(control group).Endoscopic transsphenoidal pituitary tumor resections were performed in patients with 183 cases,and 70 patients with microscopic transsphenoidal pituitary adenomas.Two groups were compared in the fields of tumor resection rate,mortality.recurrence rate,recovery of pituitary function and the incidence of complications.Results:There were no obvious differences between the two groups in sex,age,history,tumor size,and et al(P>0.05).Endoscopic endonasal transsphenoidal approach group patients total resection rate of tumor was 80.3%,subtotal removal rate was 19.7%;the whole microscope tumor resection rate was 62.9%.subtotal resection rate was 37.1%,there was no obvious significant difference between the two groups in subtotal removal rate(P?0.271>0.05).There was significant difference between the endoscopic groups(26.8%)and the microscope groups(58.6%)in major postoperative complications such as diabetes insipidus,electrolyte disorder,and hypopituitarism(P=0.0384).There was no obvious significant difference between the two groups inrecoveryofbloodhormone(P>0.05).And the endoscope group was superior to the microscope group in improving the clinical syndromes and pituitary functions(P=0.0437).The endoscope group hada lower recurrence rate than the microscope groups(P=0.0213).Conclusion:Endoscope technique is a safe way intreatmentofpituitary adenomas.It has zhe advantage of slight trauma and keep the complications low.And has a lower regrowing rate.Endoscopic transsphenoidal approach is safe,minimally invasive for the surgery of pituitary adenomas. |
