Objective A total of 516 patients with CKD 3-5 who were diagnosed with CKD were selected as the study subjects in the complete clinical data collected from The First Hospital of Qiqihar from January 2014 to December 2016.Summarized the cases of information collection,sorting and analyzing,aimed to explore the treatment of chronic kidney disease,the prevention and control to provide the reference basis,in the region the clinical features of patients with CKD 3-5.Method Selection of The First Hospital of Qiqihar kidney internal medicine diagnosed with CKD 3-5 of the hospitalized patients as the research object,the collection of patients with clinical data were retrospectively analyzed(including patients with social demographic characteristics,etiology,clinical symptoms,laboratory examination,concurrent or accompanied by disease etc.),using the software Epi Data 3.0 establish database,using SPSS 20.0 statistical software for data statistics and analysis.Result 1.Among 516 patients with chronic kidney disease,67 were20-44 years old,accounting for 13.0% of the total;202 were 45-59 years old(39.1%);247 patients above 60 years old(47.9%).And there were 282 men(54.7%)and 234 women(45.3%)in our study.2.In this survey,chronic kidney disease stage 5 has the largest number of the total,were 259(50.2%),followed by stage 3(172 patients,33.3%)and last in stage 4(85 patients,16.5).Patients over 60 have the largest number of stage 5(64.0%).3.The primary cause of chronic kidney disease is primary glomerulonephritis in this study,accounting for 46.1%,followed by diabetes(19.6%),and third in hypertension(17.2%).4.Concurrent or accompanied by disease of the top four in patients with chronic kidney disease were calcium-phosphorus metabolism disorder(83.5%),secondary parathyroid dysfunction(78.3%),hypertension(61.6%)and anemia(60.3%).5.There were a low blood pressure control rate and low glucose control rate among patients with chronic kidney disease.The prevalence rate of calciumphosphorus metabolism disorder,anemia,hypoalbuminemia and hyperuricemia increased with the chronic kidney disease aggravating.6.CKD patients over 60 have the highest mortality rate(10.9%).And the mortality rate of CKD patients combined anemia,cardiovascular disease or hyperuricemia were higher than CKD patients did not with these disease.7.Age,the level of serum albumin,e GFR and NLR is the independent influence factor of death for CKD patients(P<0.05).The level of serum albumin(OR=0.674,95%CI : 0.576-0.790)and e GFR(OR=0.675,95%CI : 0.524-0.869)were negatively correlated with death,and the level of NLR(OR=1.170,95%CI:1.038-1.319)were positively correlated with death.Conclusion The clinical data of 516 patients with Chronic Kidney Disease Stage 3-5 who were treated in a hospital in Qigihar city of hei long jiang province from January 2014 to December 2016 were analyzed retrospectively.The results were as follows: 1.In this study,the proportion of patients aged 60 years and older was the largest(47.9%),and the proportion of patients with Chronic Kidney Disease 5(64.0%)was the highest in the age of 60 years and older.The screening of the elderly with chronic kidney disease should be strengthened to find treatment as soon as possible.2.The proportion of primary diseases as diabetes and high blood pressure in patients with chronic kidney disease came in second and three only after chronic glomerulonephritis,and its control rate was low.It was necessary to strengthen the monitoring of blood sugar and blood pressure of patients with chronic kidney disease in order to reduce the harm of diabetes and high blood pressure.3.Calcium-phosphorus metabolism disorder and hyperparathyroidism were located the former two in the concurrent or accompanied by disease of chronic kidney disease.It was of great significance to regularly monitor the level of blood calcium and blood phosphorus and parathyroid hormone for improving the disease prognosis of patients with chronic kidney disease especially in chronic kidney disease stage 5 patients.4.Among patients with chronic kidney disease,the rate of anemia,low serum albumin leels,high uric acid hematic disease were not optimistic.Clinicians should increase attention to patients with chronic kidney disease who merge anemia,low serum albumin leels,high uric acid hematic disease.It was important to regular screening,standard medication,formulate individualized treatment plan for treatment and control timely.5.Age,serum albumin levels,glomerular filtration rate and grain leaching ratio level were the independent impact factors of death for patients with chronic kidney disease.It was necessary to pay attention to the treatment of elderly patients with chronic kidney disease,monitor indicators strictly in order to take timely and accurate measures to reduce the mortality of patients with chronic kidney disease.
