Application Of "One Divided Into Three" Differentiation Method In Hemorrhagic Stroke

Objectives: To explore the relation between the three syndrome types in hemorrhagic apoplexy identified by the "one divided into three" differentiation method and the six syndrome types identified by the traditional Differential Diagnosis Standards for Apoplexy,to seek the use features of the "one divided into three" differentiation method when applied to hemorrhagic apoplexy,and find out the evolution laws of syndrome type according to the symptom distribution characteristics of hemorrhagic apoplexy.Methods: Adopt the “one divided into three” differentiation method and 94 Standard to clinically conduct syndrome differentiation on the observed 114 patients with hemorrhagic apoplexy(46 in the acute phase,32 in the recovery phase,and 36 in the sequela stage)respectively on the 1st,7th,and 14 th day of hospital admission,and then analyze the results obtained by the two syndrome differentiation methods.Results:1.Conduct syndrome differentiation on the 114 cases of patients with hemorrhagic apoplexy on the first day of hospital admission by using the two kinds of syndrome differentiation methods.Among the six types of syndromes identified by 94 Standard,the wind syndrome(25.4%)and fire-heat-fire syndrome(21.1%)occupy the majority,followed by Qi deficiency syndrome(18.4%),blood stasis syndrome(16.7%),and phlegm syndrome(13.2%),while the syndrome of yin deficiency and yang hyperactivity(5.3%)have the lowest frequency.Among the three types of syndromes identified by the “one divided into three” differentiation method,the majority are patients with yang syndrome(50.1%),followed by the yin-yang miscellaneous syndrome(29.9%),while the lowest frequency appears in yin syndrome(19.3%).2.Three stages' situation of hemorrhagic apoplexy2.1 Among the 46 patients in the acute stage,in terms of six types of syndromes identified by 94 Standard,on the first day of hospital admission,the wind syndrome(52.2%)and heat-fire syndrome(19.6%)occupy the majority.With increasing days of hospital admission,the case of heat-fire syndrome and phlegm syndrome increases,and the case of wind syndrome decreases.In terms of the three syndromes identified by the “one divided into three” differentiation method,on the first day of hospital admission,the yang syndrome(50.1%)occupies the majority.With increasing days of hospital admission,the case of yang syndrome decreases,and the case of yin-yang miscellaneous syndrome increases.2.2 Among the 32 patients in the recovery stage,in terms of six types of syndromes identified by 94 Standard,on the first day of hospital admission,the heat-fire syndrome(21.9%)and phlegm syndrome(21.9%)occupy the majority.With increasing days of hospital admission,the case of blood stasis syndrome increases.In terms of the three syndromes identified by the “one divided into three” differentiation method,on the first day of hospital admission,the yin-yang miscellaneous syndrome(46.9%)and the yang syndrome(37.5%)occupy the majority.With increasing days of hospital admission,the case of yang syndrome decreases,while the case of yin-yang miscellaneous syndrome increases.2.3 Among the 36 patients in the sequela stage,in terms of six types of syndromes identified by 94 Standard,on the first day of hospital admission,the Qi deficiency syndrome(27.8%),blood stasis syndrome(25%),and heat-fire syndrome(22.2%)occur the most frequently.With increasing days of hospital admission,the case of Qi deficiency syndrome decreases,and the case of blood stasis syndrome increases.In terms of the three syndromes identified by the “one divided into three” differentiation method,their syndrome types haven't changed significantly,and the frequency of the yin-yang miscellaneous syndrome(First days 52.8%?Seventh days 55.6%?Fourteenth days 55.6%)appears the most frequently.3.The heat-fire syndrome and wind syndrome identified by 94 Standard mainly belong to the yang syndrome,while the Qi deficiency syndrome is subject to the yin syndrome,and the blood stasis syndrome,phlegm syndrome,the syndrome of yin deficiency and yang hyperactivity is part of the yin-yang miscellaneous syndrome.4.The distribution of yin syndrome,yang syndrome and the yin-yang miscellaneous syndrome,all of which are identified by the “one divided into three” differentiation method,has no significant relevance with the patient' s age and gender.Conclusion: In the syndrome differentiation diagnosis of hemorrhagic apoplexy,the three types of syndromes identified by the “one divided into three” differentiation method can basically cover the six types of syndromes identified by the 94 Standard and become a more simplified and more practical new standard.
