| Objective:In this study,Horizontal Wrist and Vertical Finger Acupuncture Manipulation was applied to acupuncture treatment for the insomnia of the liver depression forming fire syndrome(ILDFFS).The clinical effects are then compared with that of oral Chinese patent medicine Dan Zhi Xiao Yao pill treating the same insomnia by observing the variations of patients' conditions during treatment within one group that receives the same therapy as well as differences of patients' conditions after treatment between two groups that receive different therapies.This study may contribute to developing a new,safe and effective acupuncture therapy to insomnia of the ILDFFS for clinical acupuncture treatment.Methods:Total 72 cases of insomnia of the ILDFFS which met inclusion criteria were collected.These cases were then randomly divided into treatment group and control group with 36 cases in each group.Then cases in treatment group were treated with acupuncture adopted by Horizontal Wrist and Vertical Finger Acupuncture Manipulation based on the therapeutic principle of smoothing the liver,calming the sleep,purging fire and soothing the nerves.Taking the holes of Baihui(DU20),Yintang(DU29),Shenmen(HT7,bilateral),Hegu(LI4,bilateral),Taichong(LR3,bilateral),Xingjian(LR2,bilateral)and Anmian(an extraordinary points,bilateral)for acupuncture,the needles were retained for 30 minutes after deqi and were manipulated every 10 minutes,in addition to the reducing method of Horizontal Wrist and Vertical Finger Acupuncture Manipulation were applied in the Hegu(LI4),Taichong(LR3)and Xingjian(LR2),the other acupoints were applied the neutral supplementation and draining method of Horizontal Wrist and Vertical Finger Acupuncture Manipulation.One period of treatment,spanning 30 days,contains 15 therapies every other day,.Cases in control group orally took Chinese patent medicine Dan Zhi Xiao Yao pill,6g every time,twice a day.This oral treatment also spanned 30 days.All cases were carefully observed during the period of treatment.For each eligible subjects,therapeutic effect indexes,including Pittsburgh Sleep Quality Index(PSQI),Hamilton anxiety scale(HAMA),Hamilton depression scale(HAMD),and safety indexes,including blood pressure,pulse,body temperature,heart rate,electrocardiogram,blood routine,urine routine,stool routine,and function of liver and kidney,were evaluated before treatment and one day after treatment,respectively.We recorded adverse reactions and side effects occurred during the treatments.These data obtained were then analyzed using statistical software SPSS22.0 to assess efficacy and safety.Result:1.72 cases were collected initially,though eventually 67 cases completed when 5 cases dropped out.34 cases in treatment group completed(2 drop-outs)and 33 cases in control group completed(3 drop-outs).Pre-treatment statistical comparison of patients' conditions,including basic information(gender,age and course of disease),PSQI score,PSQI components,distribution of sleep quality,HAMA scores,distribution of anxiety state,HAMD scores,and distribution of depression state,revealed insignificant differences of these indexes between patients from different groups(P>0.05),indicating the two groups were comparable.2.Efficacy Evaluation: Comparison between the two group revealed that the efficacy in the treatment group is significantly better than that in the control group(P < 0.01).In addition,the cured and markedly effective rate and the total effective rate in the treatment group were 44.12% and 91.18%,respectively,significantly higher than those in the control group,which were 18.18% and 69.70%,respectively(P<0.05).3.PSQI Evaluation: The patients' PSQI total scores and each component score decreased notably in both the treatment group and the control group(P < 0.01)after treatments.However,the post-treatment total score in the treatment group was significantly lower than that in the control group(P < 0.01),and improvements on sleep quality,sleep time,sleep disorder and daytime dysfunction in the treatment group were significantly greater than those in the control group(P<0.05)as well.However,other therapeutic indicators suggested comparable changes in both group.Concerning the distribution of sleep quality,patients in both group got their sleep qualities effectively improved(P<0.01),while the improvements in the treatment group were greater than those in the control group(P<0.05).4.HAMA Evaluation: The patients' HAMA scores decreased notably in both the treatment group and the control group(P < 0.01).Besides,the reduction of scores in treatment group exceeded that in control group(P < 0.05).Concerning distribution of anxiety state,patients in both group got their anxiety level lowered(P < 0.05),and the reduction is more significant in treatment group(P < 0.05)5.HAMD Evaluation: The patients' HAMD scores decreased notably in both the treatment group and the control group(P < 0.01).Besides,the reduction of scores in treatment group exceeded that in control group(P < 0.05).Concerning distribution of depression state,patients in both group got their anxiety level lowered(P < 0.05),and the reduction is more significant in treatment group(P < 0.05)6.Safety Evaluation: all patients' blood pressure,pulse,body temperature,heart rate,electrocardiogram,blood routine,urine routine,stool routine,and function of liver and kidney were tested and then conducted before and after treatment.The results revealed no obvious changes in these physical indexes,suggesting considerable safety of the treatments.Moreover,no other adverse reactions and side effects occurred during clinical observed.Conclusion:Both acupuncture treatment adopted by Horizontal Wrist and Vertical Finger Acupuncture Manipulation and oral Chinese patent medicine Dan Zhi Xiao Yao pill exhibited noteworthy clinical effects on insomnia of ILDFFS,meanwhile they both contributed to promoting the sleep quality,reducing the sleep incubation period,prolonging the sleep time,increasing the sleep efficiency,improving sleep disorder and alleviating daytime dysfunction.However,acupuncture treatment adopted by Horizontal Wrist and Vertical Finger Acupuncture Manipulation was at an advantage in aspects of promoting the sleep quality,prolonging the sleep time,improving sleep disorder and alleviating daytime dysfunction.Both treatment methods reduced patients' total score of HAMA and HAMD,contributing to reliving anxiety and depression.Also,acupuncture treatment adopted by Horizontal Wrist and Vertical Finger Acupuncture Manipulation offered greater relief on patients' anxiety and depression.Furthermore,no obvious adverse reactions and side effects occurred during the treatments.This study provided a new,safe and effective acupuncture therapy for clinical acupuncture treatment to insomnia of the ILDFFS. |
