Objective:Observation of Yi shen jiang tang decoction on clinical stage in patients with diabetic kidney disease,and the effects on TSP-1,to further verify the clinical curative effect of Yi shen jiang tang decoction,and explore the possible mechanism of Yi shen jiang tang decoction,delaying the progression of diabetic kidney disease,and provide new ideas for the treatment of diabetic kidney disease with combination of TCM and Western medicine.Methods:88 cases of diabetic kidney disease with Qi and yin deficiency and blood stasis syndrome were randomly divided into two groups.The control group(n=44)and the test group(n=44).The control group was treated with simple Western medicine,and the treatment group was given Yi shen Jiangtang Yin on the basis of the control group.The two courses of treatment both were 60 days.Before and after treatment,the comprehensive clinical curative effect and TCM syndrome curative effect were observed.The changes of TSP-1,ACR,24h UV,FGB,blood lipid and renal function before and after treatment were compared with those of traditional Chinese medicine syndromes,And the results of the relevant statistical analysis.Results:1.The two groups of patients before treatment,such as general information,such as gender,age,duration of disease,related clinical biochemical indicators,TCM syndrome scores were no significant difference(P>0.05),comparable.2.The levels of FPG,2hPG,TC,TG LDL-C,ACR and 24h urinary protein in the two groups after treatment were decreased,Scr decreased,eGFR increased,the difference was statistically significant(P<0.05).3.Treatment group and control group serum TSP-1 level of two groups was obviously higher than that of healthy control group After treatment,blood TSP-1 in both groups decreased compared with that before treatment(P<0.05),and the experimental grouP was better than the control group(P<0.05).4.Correlation analysis:Blood TSP-1 was positively correlated with FGB,2hPG,and HbAlc(P<0.05).TSP-1 was positively correlated with ACR,24hUV,Scr,and negatively correlated with eGFR(P<0.05)..5.The total effective rate of TCM syndromes in experimental groupand control group was 88.09%and 67.44%,respectively,with statistical significance(P<0.05).6.The experimental group and control group clinical comprehensive efficacy were 83.34%and 62.79%,the difference was statistically significant(P<0.05).Conclusion:1.Yi shen Jiang tang Decoction significantly improves the clinical symptoms of patients with stage IV diarrhea and diarrhea in Qi and Yin deficiency,and the curative effect is better than that of western medicine alone.2.The blood glucose,blood lipids,urinary protein,and renal function of the two groups were improved after treatment.The treatment group was superior to the control group in terms of curative effect,indicating that the combination of Yishui Jiangtang Yin was superior in reducing urinary protein excretion and regulating glucose and lipid metabolism disorders.Control group.3.Compared with the healthy group,serum TSP-1 levels increased in patients with stage IV diarrhea and type 2 diabetes mellitus with qi and yang deficiency;FGB,2hPG,HbAlc and TSP-1 were positively correlated,and TSP-1 and ACR were 24 hUV.Positive correlation was found between Scr and eGFR,suggesting that TSP-1 level is closely related to the progression of DKD.4.Yishui Jiangtang Decoction may reduce urinary protein and delay the progression of renal function by decreasing the expression of TSP-1.
