Study On The Correlation Between Somatization Disorder And Depression

Objective Depression can induce somatic diseases and aggravate the pain experience of somatic diseases,and somatic diseases can be the direct result of emotional problems.Depression is related to the incidence of myocardial infarction and cancer.Somatization disorder is a common disease characterized by multiple,recurrent and frequently changing somatic symptoms.It is a common disease with high morbidity,often accompanied by obvious depression and anxiety.This study mainly discussed the correlation analysis between the somatization disorder and depression of the prisoner of heavy sentence,further explored the characteristics of somatization disorder and depressive disorder in prisoners,and tried to provide objective evidences for the diagnosis of somatization disorder,improved the quality of prisoners' reform,avoided the waste of medical resources in the prison.Method This study took prisoners in a prison in Jiangsu Province as the research object from January 2014 to February 2017.The three general practitioners with professional knowledge and rich clinical experience in the field of psychiatry screened and diagnosed of somatization disorder and depression in prisoners.All patients with somatization disorder met the diagnostic criteria of "somatization disorder" in Chinese psychiatric classification and diagnostic criteria(CCMD-3).All patients with depression met the diagnostic criteria of depression in CCMD-3.Finally,55 cases of somatization disorder and 55 cases of depression were included.All the patients were male,aged between 21 and 55 years old,all above were primary school education level,and all the enrolled members signed the informed consent form.Used questionnaire,Hamilton depression rating scale,Eysenck personality questionnaire to collect information about patients,and used SPSS19.0 statistical software to compare and analyze the indicators between two groups of somatization disorder and depression patients.The quantitative data were analyzed by t test,the qualitative data were analyzed by chi-square test,and the correlation among the main indicators between somatization disorder and depression patient was analyzed by single factor and multiple factors correlation analysis method.Results 1.Somatization disorder group and depression group in age,(before arrested)occupation,marital status,family economic situation had no significant difference,but there was a significant difference between the somatization disorder group and the depression group in residence,the rural household registration rate in somatization disorder group was significantly higher than that in depression group(70.91% vs.38.18%,P=0.001).The age of onset in somatization disorder group was significantly lower than that in depression group(P=0.041);At the same time,the course of patients with somatization disorder were significantly longer than those with depression(P < 0.0001);The predisposing factors were statistically significant differences between somatization disorder group and depression group(P=0.003),The morbidity rate of somatic diseases in somatization disorder group was significantly higher than that in depression group(34.55% vs.10.91%),the incidence of psychological stress events in patients with depression was significantly higher than that in patients with somatization disorder(78.18% vs.47.27%),the family history rate of depression in depression group was significantly higher than that in somatization group(36.36% vs.12.73%,P=0.004).The age of imprisonment in somatization disorder group was significantly higher than that in depression group(P<0.001).2.The system number of complaints in patients with somatization disorder involved in somatization disorder group were(3.69±1.46),the total number of discomfort in complaints were(6.05±1.37);In the somatization disorder group,38 cases patients with complaints of the nervous system,48 cases patients with complaints of skin system,36 patients with complaints of breathing system,44 cases patients with complaints of circulatory system,35 cases patients with complaints of musculoskeletal system,38 cases patients with complaints of urinary system,33 cases patients with complaints of reproductive system,36 cases patients with complaints of digestive system.The system number of complaints in patients with somatization disorder involved in depression group were(2.44±1.03),the total number of discomfort in complaints were(2.80±1.21);In the depression group,16 cases patients with complaints of the nervous system,19 cases patients with complaints of skin system,22 patients with complaints of breathing system,17 cases patients with complaints of circulatory system,27 cases patients with complaints of musculoskeletal system,17 cases patients with complaints of urinary system,22 cases patients with complaints of reproductive system,25 cases patients with complaints of digestive system.The system number of complaints,the total number of discomfort complaints and complaints of each system in somatization disorder group were significantly higher than that of patients group with depression.3.Somatization score(P=0.035)and obsessive score(P=0.020)of somatization disorder group were significantly higher than those of patients group with depression;Sensitivity score(P=0.022),depression score(P=0.002),terror score(P=0.029),paranoid score(P=0.045),psychiatric score(P=0.048),and SCL-90 score(P=0.042)of somatization disorder group were significantly lower than those of patients group with depression;There was no significant difference in anxiety score(P=0.102)and hostility score(P=0.686)between somatization disorder group and depression group.4.Somatization disorder patients only characterized by mild depression,the depression group patients are characterized by moderate to severe depression,somatization disorder patients HAMD scale total score was significantly lower than the total score of patients group with depression(P < 0.001);The five factors of HAMD scale showed that somatization factor score in somatization disorder group was significantly higher than that of depression group(P=0.003);The weight factor score of somatization disorder group was significantly lower than that in depression group(P < 0.001);the cognitive impairment factor of somatization disorder group was significantly lower than that in depression group(P < 0.001);the retardation factor of somatization disorder group was significantly lower than that in depression group(P < 0.001);There was no significant difference of sleep factor between somatization disorder group and depression group(P = 0.580).5.The EPQ-E scores of patients group with somatization disorder were significantly higher than those in patients group with depression(P = 0.043);the EPQ-N scores of patients group with somatization disorder were significantly higher than those in patients group with depression(P = 0.013);the EPQ-P scores of patients group with somatization disorder were significantly lower than those in patients group with depression(P = 0.022);the EPQ-L scores of patients group with somatization disorder were higher than those in patients group with depression(P = 0.035).Conclusion 1.Somatization disorder and depression are similar in general clinical data,such as age,occupation,marital status,education year,and family economic situation,but there is a significant difference about residence between the somatization disorder group and the depression group.Somatization disorder patients are mainly chronic onset,chronic course of disease,Most of the patients with depression were acute and subacute onset,the course of disease was short and most of them were paroxysmal.2.Somatic disorder patients mostly have systemic and changeable somatic discomfort symptoms,and their symptoms are more common than depression,and the range of symptoms involved is wider than that of depression.3.Somatic disorder patients have a better overall mental state than those with depression,but somatization disorder patients have a variety of somatic symptoms,which can lead to a variety of complex psychological changes and mental symptoms.There is a certain similarity between somatization disorder and depression with specific psychological problems,there are also statistical differences.Somatization disorder is concerned with somatic symptoms and compulsion,while depressed patients are interpersonal sensitive,suspicious,paranoid,mental psychology.4.Somatization disorder group and depression group both have depressive emotion,somatization disorder patients showed mild depression,more with depression and anxiety,depression patients showed by moderate to severe depression.5.Depressive patients almost have the characteristics of solitary,enjoyed being alone,insensitive,hostile to people,suspicious and so on.Patients with somatization disorder have the characteristics of extroversion,high emotion,sensitive,friendly to others and so on.Patients with somatization disorder prefer to express and confide in their personalities more than those with depression,and often talk to the outside world about their feelings about physical discomfort,and have strong dependence,more inclined to cover up psychological and emotional problems,personality characteristics are consistent with clinical features.
