Objective:To study the Perioperative blood coagulation dysfunction influence on the prognosis of neurosurgical patients.Methods:1.122 patients admitted to the hospital emergency and postoperation TBI patients,instant determination of plasma activated partial admission clotting enzyme live time(APTT),prothrombin time(PT),thrombin time(TT),fibrinogen(FIB),the international standardization ratio(INR),platelet(PLT),to compare coagulant function change with different degree of craniocerebral injury.2.Carrying on the dynamic review CT to the TBI patients after admission(6 hours,12 hours,24 hours a day,3 days),observe whether progress bleeding and postoperative bleeding again;3.According to with or without blood coagulation dysfunction,divided into normal group(group A)and abnormal group(group B),to compare the incidence of Progressive Hemorrhagic injury(PPI)and Postoperative hemorrhage(PPH);4.For all TBI patients were followed up for 6 months,GOS score,grouping comparing the difference between the blood coagulation function relationship with the prognosis.Results:1.Comparing to the group A,the group B have a higher incidence of PPI and PPH,and the difference is significantly(P?0.05);2.when postoperative,Hb and PLT is decreased obviously compare with admission(P?0.05),compared with postoperative,APTT and PT extend(P?0.05),FIB is significantly decreased in postoperative(P?0.05).The PPH correlate with extension of APTT,TT and low levels of FIB(P?0.05).3.The prognosis of patients with TBI patients admitted to hospital with Hb,FIB levels were positively correlated(P?0.05),and the TT when patients admitted to hospital,PT and INR negatively correlated(P?0.05);Prognosis of TBI patients with postoperative instant review of Hb,FIB levels were positively correlated(P?0.05),and negatively correlated with postoperative instant review of TT,PT(P?0.05).4.Group B with TBI patients have lower GOS score than the group A,and the difference is significantly(P?0.05);Conclusions:1.More than four abnormal indicators of coagulation function,PHI and PPH was significantly higher incidence,3 and 3 the following abnormal indicators coagulation function,PHI and PPH incidence was not significantly increased;2.TBI patients who coagulation abnormalities had a significant impact on prognosis,their the mortality rate,severe disability was significantly increased.
