Objective To investigate the use of kangaroo mother care(KMC)in extremely low birth weight babies and its effect on mothers,nurses who participated in the KMC.Methods 160 extremely low birth weight babies born in Nanjing maternity and child health hospital were rolled in this study,and randomly divided into two groups:control group(n=80)and intervention group(n=80).The control group was treated with preterm infant care routine,and the intervention group was treated with KMC on the basis of preterm infant care.The weight,head circumference,length,time of hospitalization,hospitalization cost and breast-feeding rate,complications occurred during the hospital stay were compared between the two groups.And compared the maternal anxiety and depressi,the basic nursing situation,the lactation yield of the mothers in the two groups,and Compare nurses' workload and enthusiasm before and after kangaroo nursing.Resluts1.The daily weight of babies in the intervention group were significantly higher than that in the control group(4.16 + 0.87vs3.75 + 0.54,P<0.05).The daily growth rate of length in the intervention group was also significantly higher than that in the control group(1.1±0.25vs0.87±0.34,P<0.05).There was no significant difference in the growth rate of head circumference between the intervention group and the control group(1.26 + 0.46vs0.98+ 0.23,P>0.05).2.The proportion of children with breast milk in the intervention group reached 50 to 80%higher than the control group(31.25%vs22.5%,P<0.05).Compared with the control group,the proportion of children with breast milk that reached 80%was greater(P<0.05).The total rate of breast proportion of intervention group was significantly higher than the control group(70.8%vs54.28%,P<0.05).The proportion of preterm infants who were exclusively breastfed during the hospitalization was significantly higher than that in the control group(78.65%vs40.90%,P<0.05).3.The incidence of NEC,septicemia,pneumonia and feeding intolerance in the intervention group were significantly lower than that in the control group(1.25%vs5%,P=0.002;10%vs 18.75%,P = 0.024;15%vs 21.25%,P = 0.033;35%vs52.5%,P = 0.017).4.The hospitalization time of the children in the intervention group was significantly shorter than that in the control group(12.38±3.57 ? vs16.45±4.28 ?,P<0.05;Compared with the control group,the final hospitalization cost of the children in the intervention group was significantly reduced(6.75 + 2.14 vs 8.28 + 2.34,P<0.05).5.By evaluating the effect of KMC on mothers,the depression SDS score of children in the intervention group was significantly lower than that in the control group(31.35±3.86vs44.28±4.50,P<0.05).The SAS score of intervention group was also significantly lower than the control group(33.78 + 3.24vs38.25 + + 3.67,P<0.05).In the intervention group,the matery of knowledge of basic nursing of neonates was higher than the control group(90%vs70%,P<0.05).Further comparison of the effects of KMC on maternal lactation,the proportion of maternal lactation in the intervention group was significantly higher than that in the control group(60%vs47.55%,P<0.05).6.After the implementation of KMC nurses work enthusiasm on career were higher than that before the implementation of KMC(P<0.05),The NAS scores found that after the implementation of KMC the nursing workload was significantly reduced than before(63.28±6.75vs52.16±7.54,P<0.05)?7.Pearson and Spearman correlation analysis reveals that KMC intervention care was negatively correlated with the average hospitalization time(r =-0.312,P= 0.03).Apgar score(r=0.354,P= 0.04),gestational age(r = 0.332,P= 0.06)was positively correlated with the average time of hospitalization of babies.Conclusions1.KMC can promote the growth and development of extremely low birth weight babies,improve the rate of breastfeeding,shorten hospital stay,and reduce hospitalization expenses.2.kangaroo care can ease the worries,depression of the mothers,improve the basis nursing skills of very low birth weight children,promote the mother milk secretion.3.The KMC for the extremely low birth weight can reduce the burden of nurses and improve the enthusiasm of nurses in the long run.
