Abstract/Summary:
Objective: We designed a multicentric,prospective study to confirm PCT as the predictor of AL in patients diagnosed wtih colorectal cancer;To assess if PCT can be used as one of the early discharge criteria of colorectal cancer in patients within ERAS program.Methods: Patients underwent colorectal cancer surgery were included in Hunan Tumor Hospital and Hunan Provincial People’s Hospital.PCT,WBC,T,HR levels were measured at postoperative days 1,3,5 after colorectal surgery.31 patients with AL were divided into experimental group,and 51 cases with Non-AL were chosen as control group by paired design.Results: 1.No statistics differences in tumor stage,tumor position,surgical approachs,ERAS program,chronic obstruction between the two groups(P > 0.05);2.Preoperative hypoalbuminemia was a independent risk factor of AL(OR= 2.74,95%CI(2.12,3.36),P < 0.05));3.The PCT,WBC,T and HR levels has increased on postoperative day1(POD1),POD3 and POD5 in all patients(P<0.05,P of WBC,T,HR in POD 1 were greater than 0.05);4.On analysis of ROC curves,the specificity,accuracy and sensitivity of PCT was higher than that of WBC,T and HR on PODs;5.The cut-off for PCT on POD 1 was 0.76ng/ml,with the highest sensitivity((94.6%)for AL.The cut-off for PCT on POD 5 was 4.58 ng/ml,with highest specificity(93.1%)for AL.Conclusions: 1.PCT continuous measurement on postoperative day can become the predictor of AL in CRC patients;2.Compared with the test of WBC,T and HR,PCT has higher sensitivity and specificity in the prediction of AL;3.The continuous low-level of PCT on postoperative day can be used as one of the early discharge criteria of CRC patients within an ERAS program.
