| Objective:The aim of this study was to detect the expression levels of 8-Hydroxy-2’-deoxyguanosine(8-OHdG)in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD),to explore the clinical significance of 8-OHdG in AECOPD.Methods:121 patients with AECOPD in the respiration department of our hospital from September 2015 to August 2016 were collected as AECOPD group,28 with COPD in stable period were served as COPD stabilization group,while 20 healthy people underwent physical examination were served as healthy control group.8-OHdQ neutrophil CD64 average fluorescence intensity index(CD64 index),procalcitonin(PCT),C-reaction protein(CRP)in all groups were detected,lung function parameters were also examined.Simultaneously,Modified Medical Research Council(mMRC)dyspnea score and COPD Assessment Test(CAT)were retrospectively recorded.All the data for the statistical analysis by SPSS 18.0 software.Results:1.8-OHdG levels in AECOPD group was higher than COPD stabilization group(P<0.05),there was no statistically difference between COPD stabilization group and control group(P>0.05).The levels of 8-OHdG,CD64 index,PCT and CRP in AECOPD group were significantly higher than those in control group with statistical differences(P<0.05).2.In AECOPD group,8-OHdG levels was significantly higher in patients with GOLD 3,4 grade than that of GOLD 1 grade(P<0.05),and 8-OHdG levels was higher in patients with GOLD 4 grade than GOLD 2 grade(P<0.05).3.Ex-and current smokers in AECOPD group exhibited increased expression of 8-OHdG in comparison with nonsmokers(P<0.05);however,there was no significant difference between ex-and current smokers(P=0.64).And the higher smoking index was,the greater the level of 8-OHdG was(P<0.05).4.In the univariate analysis,there were positive correlations between 8-OHdG and CD64,PCT and CRP(r=0.423,r=0.276,r=0.325,P<0.05);and negative correlations between 8-OHdG and FEV1,FEV1%pred and FEV1/FVC(r=-0.25,r=-0.22,r=-0.242,P<0.05).5.8-OHdG levels tend to be higher in AECOPD patients coexist with associated comorbidities,such as lung cancer,diabetes and cardiovascular diseases.6.Compared with mMRC grades 0,1,2 patients,8-OHdG values were markedly elevated in mMRC grade 4 patients(P<0.01);and 8-OHdG levels were higher in mMRC grades 2,3,4 patients in comparison to mMRC grade 1 patients(P<0.05).Moreover,there were positive correlations between 8-OHdG and CAT scores(r=0.419,P<0.05).Conclusion:8-OHdG increased in AECOPD patients,it can be used as the diagnostic marker for AECOPD patients,its levels may reflect the lung function,smoking status,comorbidities,mMRC grades and CAT scores before AECOPD,and guide to chose comprehensive therapeutic decision. |
