ObjectiveThe aim of the prospective clinical trial was to analysis the VEEG(Video electroencephalogram)monitoringand REEG(Routine electroencephalogram)monitoring in patients with sleep-related epilepsy by compared results of VEEG and REEG.The temporal distribution characteristics of electric physiological activities,such as they background EEG(electroencephalogram),the interictal epileptiform discharge and the epileptic seizure,were studied.And we observed the clinical manifestations and explored the pathological and physiological characteristics of sleep-related epilepsy.To evaluate the diagnostic value of VEEG for sleep-related epilepsy,so as to further reduce the missed diagnosis and misdiagnosis rate,and it's helpful to identify the type of seizure,to diagnose epilepsy syndrome,and to guide the delivery time,etc.MethodWe collected the clinical manifestations and history of patients with epilepsy in Nanjing Brain Hospital from November 2014 to July 2013.And 119 cases of them with sleep-related epilepsy were selected.These 119 sleep-related epilepsy patients were treated by VEEG and REEG for self-contrast.Based on the video monitoring data and retrospective analysis of EEG data,we analyzed the background EEG,interictal epileptiform discharge and clinical seizures.REEG and VEEG data were comparative analyzed with statistical.ResultThe total abnormal rate of epilepsy was 33.61%by REEG monitoring,and was 98.32%by VEEG monitoring.There had inportant difference between the two methods.x2= 100.83,P<0.05.The detection rate of epileptoid discharge was 18.49%by Routine-EEG monitoring,and was 85.71%by Video-EEG monitoring.There had inportant difference between the two methods.x2= 107.75,P<0.05.No clinical events were recorded with REEG monitoring of these 119 cases.Six patients were diagnosed as BECT(benign epilepsy of childhood with the central superior temporal area spike wave)by VEEG.The clinical events were recorded in 42 patients during the VEEG monitoring,and 1 case were SED(subclinical epileptogemi discharge),seizures for 41 cases.To observe this 41 cases of patients with onset interval EEG,6 cases of seizure interval were monitored to the seizures in both wake,sleep periods,and 34 cases were monitored to the seizures only on sleep periods.ConclusionVEEG monitoring can be integrated to collect EEG data and clinical video data of wake-sleep-wake cycle.The clinical manifestations and electrophysiological activity of Sleep-related epilepsy were relative hidden,so VEEG can improve the detection rate of background anomaly,interictal epileptiform activity and epileptic seizure.VEEG can record the clinical manifestations of sleep-related epilepsy in patients with epilepsy,and provide a more direct and effective data for the clinical symptoms of epilepsy.VEEG has important value in the diagnosis of the diagnosis of epilepsy syndrome,the type of seizure,and the determination of treatment plan.
