Part I The leveland related factors of serum ANGPTL3 in patients with metabolic syndrome and type 2 diabetes mellitusObjectiveTo evaluate the level and correlative factors of serum ANGPTL3 in patients with metabolic syndrome and type 2 diabetes.MethodsA total of 150 subjects were selected from aged 40 to 65 years old in Shanghai area from May,2015 to April,2016.Using diagnostic criteria for the metabolic syndrome in the 2013 edition of the Chinese Medical Association Diabetes Society(CDS),A total of 150 subjectswere divided into 83 subjects with non metabolic syndrome group and 67 patients with metabolic syndrome,and according to the 1999 WHO diagnostic criteria for diabetes,the objects of study were divided into 49 cases of normal glucose regulation as control group and 101 cases of diabetes mellitus group.The fasting serum concentration of ANGPTL3 was detected by liquid phase factor concentration quantitative analysis(Luminex).Waist circumference,hip circumference,blood Pressure,blood biochemistry,and glycometabolism indicators were also measured.Using SPSS 21.0 software for statistical analysis.Results1.Serum level of ANGPTL3 was significantly higher in MS group(3.40±1.60)ng/ml than in NMS group(2.66±1.33)ng/ml(P=0.001).2.Serum ANGPTL3 levels of T2 DM group(3.17±1.46)ng/ml was also higher thanthat of control group(2.62±1.51)ng/ml(P=0.026).3.Spearman correlation analysis indicated that serum ANGPTL3 was positively associated with waist circumference,WHR,BMI,triglycerides,fasting blood glucose,fasting insulin,HOMA-IR(P<0.05).4.Multiple linear stepwise regression analysis showed that the triglyceride was an independent risk factors for serum ANGPTL3 level afteradjusting for the effects of BMI,waist circumference,WHR,fasting glucose,fasting insulin,HOMA-IR,triglyceride,Conclusions1.Serum level of ANGPTL3 was elevated in the patients with metabolicSyndrome and type 2 diabetes mellitus2.Triglyceride was independently correlated with pressure,blood biochemistry,and glucose metabolic index were also measured.SPSS 21.0 statistical software was used in all analysis.Results1.serum ANGPTL4 level in male(9.42±4.18)ng/ml was higher than that of femal(6.94±2.22)ng/ml(P<0.01).2.The serum ANGPTL4 level in the metabolic syndrome group(8.92±4.22)ng/ml was significantly higher than that in the non metabolic syndrome group [(7.41±2.54)ng/ml,P=0.008].The serum ANGPTL4 level(10.85±4.81)ng/ml in male MS group was significantly higher than that in male non MS group(7.99±2.84)ng/ml(P=0.003).The female serum levels of ANGPTL4 in the MS group and the non MS group were(6.99±2.32)ng/ml and(6.90±2.15)ng/ml,P=0.867,the difference was not statistically significant.3.Serum ANGPTL4 levels of T2 DM group(8.69±3.79)ng/ml was also higher thanthat of NGR group(7.03±2.61)ng/ml(P=0.006).The level of male serum ANGPTL4(10.45±4.32)ng/ml in type 2 diabetic patients was significantly higher than that in NGR group(7.36±3.02)ng/ml(P=0.003).The level of female serum ANGPTL4 of type 2 diabetes mellitus and normal glucose regulation group were respectively(7.06±2.25)ng/ml and(6.72±2.18)ng/ml,the difference was not statistically significant.4.The level of serum ANGPTL4 in different age groups: compared with the 40~years old group,the serum ANGPTL4 level was statistically significant in the 50~years old group and 60~years old group,but there was no significant difference between the 50~years old group and the 60~years old group.The serum ANGPTL4 level of men and women in all age groups:the level of male serum ANGPTL4 was significantly higher than that of female in aged 50~years old group and 60~years old group,but there was no significant difference in serum ANGPTL4 level between male and female in the 40~years old group.5.Spearman correlation analysis indicated that serum ANGPTL4 was associated with Gender?age?BMI?waist circumference?Waist to hip ratio?r-glutamine?low density lipoprotein?high density lipoprotein?fasting blood glucose?2 hour postprandial blood glucose?glycosylated hemoglobin?HOMA-IR(P<0.05).6.Furthermore,multiple linear stepwise regression analysis showed that high density lipoprotein and 2 hour postprandial blood glucose were independent risk factors for serum ANGPTL4 level after adjusting for the effects of Gender and age(? are respectively-3.047 and 0.205,(P<0.05).ConclusionSerum level of ANGPTL4 was elevated in the patients withmetabolic syndrome and type 2 diabetes mellitus.It was associated with glucose and lipid metabolism and HOMA-IR?body fat distribution,which indicate that ANGPTL4 may play a role in the pathophysiology of diabetes and metabolic syndrome.The serum ANGPTL4 levels in male patients with metabolic syndrome and type 2 diabetes were significantly higher than those in women,and there was no difference in serum ANGPTL4 levels between the two groups.
