Objectives: The purpose of this study is to detect the changes of serum nerve growth factor(NGF)and the Montreal score(MoCA)before and after comprehensive treatment in patients with severe obstructive sleep apnea hypopnea syndrome,and to analyze the correlation among serum NGF level,MoCA score,Apnea hypopnea index(AHI),lowest oxygen saturation(LSaO2),and to explore the predictive value of NGF for the risk of cognitive impairment in patients with severe OSAHS.To assess the effect of CPAP combined with surgery on severe OSAHS patients.Method: Among the patients diagnosed with otorhinolaryngology of Hunan Provincial People's Hospital from October 2015 to October 2016,80 patients with severe OSAHS aged 41-60 years were selected as subjects(According to the 2011 Chinese Medical Association to develop OSAHS diagnostic criteria),to evaluate the Montreal rating.All the subjects must complete routine laboratory tests,and exclude other hypoxic diseases.The next morning the upper serum of the patients' venous blood were absorbed after centrifugation,and then frozen at refrigerator which temperature setted at-80?.The patients' serum NGF levels were measured by enzyme-linked immunosorbent assay(ELISA).The patients were treated with CPAP for 5 days combined with H-UPPP or H-UPPP plus tongue-based plasma ablation.After 3 months,we lost 15 patients' datas.65 followers were followed by MoCA score,PSG monitoring and extraction of elbow vein blood.And the serum NGF levels were measured by ELISA in 65 patients with severe OSAHS.SPSS21.0 statistical software was used to analyze the changes of serum NGF and MoCA scores in 65 OSAHS patients before and after treatment,and the correlation among serum NGF level,MoCA score,AHI,LSaO2 in severe OSAHS patients.Results:1.Contrast the levels of the severe OSAHS patients before and after three months of the treatment,the levels of serum NGF were 33.27 ± 0.56 ng / ml and 52.24 ± 2.69 ng / ml,respectively.The difference between the two groups was statistically significant.MoCA score was 24.38 ± 1.05 points and 26.04 ± 1.09 points,respectively;the difference between the two groups was statistically significant as well.2.There was a positive correlation between serum NGF level and MoCA score in severe OSAHS patients(r = 0.663,P <0.01),and negatively correlated with AHI(r =-0.648,P <0.01),and positively correlated with LSaO2(r = 0.692,P <0.01),respectively.There was a negative correlation between the MoCA score and AHI(r =-0.614,P <0.01),and positively correlated with LSaO2(r = 0.634,P <0.01),respectively.Conclusion:1.The serum NGF levels and cognitive function decrease in patients with severe OSAHS.2.Combined treatment of surgery plus CPAP can increase serum NGF levels and improve cognitive impairment in patients with severe OSAHS.
