Objective:To investigate the clinical characteristics of early intracranial hemorrhage(ICH)in acute non-promyelocytic leukemia(ANPL),and to explore the high risk factors of early ICH in ANPL,and to provide a strategy for early prevention and treatment of ICH.Methods:A total of 1467 patients with ANPL who underwent continuous treatment from January 2010 to April 2016 in Zhejiang University Medical College First Affiliated Hospital Department of Hematology were enrolled in this study.Ninety patients with ICH were identified as experimental group,and 139 patients without intracranial hemorrhage(NICH)of the ANPL patients were randomly selected as the control group.To compare the gender,age,FAB classification,blood routine index,blood biochemical index,hemagglutination index,previous history and family history of the two groups,and to explore the risk factors of early ICH.Results:Compared with group NICH,group ICH had higher level of white blood cell(WBC)at first visit(86.63±98.26×109/L vs 33.04±51.48×109/L,P<0.001),lower level of platelet(PLT)(36.14±30.68×109/L vs 58.46±53.96×109/L,P<0.001),lower level of albumin(ALB)(35.77±4.71g/L vs 39.14±5.26g/L,P<0.001),lower level of fibrinogen(FIB)(2.15±1.43g/L vs 3.22±1.21g/L,P<0.001),and higher prothrombin time(PT)(14.01±2.32 seconds vs 12.19±1.62 seconds,P<0.001);single factor analysis WBC?30.00×109/L(P=0.001,HR=2.108,95%CI 1.377?3.228),FIB<2.92g/L(P=0.029,HR=1.625,95%CI 1.052?2.510),PT? 12.91 seconds(P<0.001,HR= 2.293,95%CI 1.468?3.583)Intra-hemorrhage risk factors;multivariate analysis,WBC?30.00×109/L(P=0.045,HR=1.600,95%CI 1.011?2.532),PT? 12.91 seconds(P=0.010,HR=1.877,95%CI 1,164?3.026)are ANPL early intracranial hemorrhage independent risk factors.Conclusions:WBC?30.00×109/L and PT?12.91 seconds were independent risk factors of early ICH in ANPL,for newly diagnosed high white blood cells and coagulation dysfunction in patients with ANPL,should pay attention to prevent the occurrence of ICH.
