Objective To discuss the necessity of establishing the case data acquisition system of prostate cancer in the clinical work,summarize the experience of the design,use,input,output,processing and analysis of the data when applicating to practice.To understand the basic information and follow-up of patients with newly diagnosed prostate cancer in our hospital,evaluate the role of the system and prospect the follow-up and data management of other related fields in urology,we analyze the relevant data of our hospital's new cases of prostate cancer patients from 2012 to 2016.Methods According to the needs of patients and clinical medicine,we cooperate with the company to develop the new system case data acquisition system for Prostate cancer patients based on the Internet.The relevant information such as laboratory examination,image data,new complications,diseases and other related information were collected before and during the follow-up of the newly diagnosed patients,then input,display,output and analyze the data.Through retrospectively analyzing the clinical data and follow-up data,we discussed and summarized the changes of related data of prostate cancer patients and apply the system intoinformation management and follow-up of patients with prostate cancer in the past 5 years.Results The system included the personal data,clinical information and follow-up information of 523 patients with newly diagnosed prostate cancer.Through a retrospective analysis,we can learn the following results.(1)In the past 5 years,the number of new cases of prostate cancer in our hospital has shown a increasing trend.The age ranges from 48 to 95 years old,with an average age of 73.08 years.The average age range of new onset patients each year varied from 72.3 to 73.4 years,and the median range varies from 72.3 to 73.4 years.(2)Serum PSA was determined in 516 patients,with a minimum value of 0.05ng/ml and a maximum value of >10000.00 ng/ml.In 22 patients,PSA was less than 4 ng/ml,and PSA exceeded 100 ng/ml in 133 patients.When serum PSA was less than 100ng/ml,the number of prostate cancer patients decreased with increasing serum PSA.(3)In the past 5 years,the most common cause of new prostate cancer seek for medical treatment is lower urinary tract symptoms,followed by abnormal serum PSA.Fewer patients were visited because of osteodynia,hematuria or other causes.Patients with abnormal PSA values were increasing in recent years.(4)Among the methods of diagnosis of prostate cancer,485(92.73%)patients were confirmed by pathological diagnosis of prostate biopsy.24(4.59%)patients were confirmed by the pathological diagnosis after transurethral resection of prostate or prostatectomy.8(1.53%)patients were confirmed by postoperative pathology of bladder tumor.6(1.15%)patients were diagnosed on the basis of PSA and imaging data.(5)Of the 523 patients who received the first treatment,200 were treated with radical prostatectomy combined with endocrine therapy,7 treated with neoadjuvant endocrine therapy before prostatectomy.105 cases received simple endocrine therapy,68 cases received radioactive particle implantation and endocrine therapy,2 cases underwent operation castration,and 2 cases received chemotherapy.(6)127 cases of patients diagnosed with existing bone metastasis,and 41 cases of patients could not be assessed due to lack of data.There were 26 cases with stage T1 and 1 with bone metastasis,271 cases with stage T2 and 37 with bone metastasis,122 cases with stage T3 and 46 with bone metastasis,63 cases with stage T4 and 36 with bone metastasis.(7)Among the Gleason score of 503 Cases(96.18%),the minimum score was 5 points,the maximum score was 10 points,including excellently differentiated(Gleason score 2 to 6)in 178 cases,moderately differentiated(7 Gleason score)in 174 cases,poorly differentiation(Gleason score 8 to 10)in 151 cases.(8)The preoperative serum PSA values were positively correlated with the Gleason scores of the puncture pathology and the postoperative pathology.There was no consistency between the Gleason scores of the puncture specimens and the radical specimens.Conclusion The new follow-up system can store information quickly and effectively,manage the relevant data and image data of patients with prostate cancer systematically,which is also easily used and the data is exported and calculated quickly and flexibly.By using this follow-up system,we have a general understanding of the information about the new prostate cancer patients in our department of urology during the past 5 years.It is of great significance for the practical application of information management,clinical decision making and medical science research.
