Objective:The multiple organ dysfunction syndrome(MODS)of severe trauma,including burn(scalded),involved in infection,inflammation,oxidative stress and blood coagulation dysfunction etc.,and Chinese combined western medicine therapy could be the important way to treat severe traumatic MODS.We studied the effects of treatment of Tetramethylpyrazine(TMP)and insulin on the infection,inflammation,oxidative stress and blood coagulation dysfunction in the Animal models of severely scalded MODS rats,in order to provide a experimental basis for Tetramethylpyrazine and insulin as effective prevention and treatment drugs for the severely traumatic MODS.Method:1)30% TBSA III degree burns MODS rats,as a severe trauma animal model of MODS,were made by intraperitoneal injection of endotoxin after severe skin scalded.The 30% TBSA III degree burns MODS rats were randomly divided with random number method into Tetramethylpyrazine(TMP)treatment group(TMP group,n =20),insulin treatment group(INS group,n = 20),Tetramethylpyrazine and insulin combined treatment group(TMP + INS group,n = 20)and the control group(C group,n = 20).2)The changes of index of liver dysfunction(ALT),renal dysfunction(CRE)and myocardial enzyme(CK)in the the Different groups of severe scalded rats at the different time of before injury and 1,3,5,7 days after injury were observed to confirm the success of the animal model of trauma MODS.3)The death rate of different groups of severe scalded MODS rats in 7 days were observed and recorded.4)The levels of the blood sTREM-1,sICAM-1,MDA,SOD,D-2G,and FDP in different groups before and after the injury were measured.5)The data of mortality,plasma GLU,sTREM-1,sICAM 1,MDA,SOD,D-2G,and FDP index level in TMP group,insulin group,TMP and insulin group andcontrol group,were statistical analyzed using Medical statistical software SPSS11.0,and P < 0.05 represented statistically significant.Results:1)The levels of the fasting blood glucose,ALT,CRE,and CK injury in rats of C group at the time of after injure 1,3,5,and 7 days were significantly higher than before injure(P < 0.01),and continue for 7 days after injury,the mortality in 7 days was 20%,which testified the 30% TBSA III degree burns MODS rats used by this study were successful experimental traumatic MODS animal models.2)The levels of the fasting blood GLU,sTREM-1,sICAM-1,MDA,D-2G,and FDP in rats of C group at different time after injure were significantly higher than before injure(P < 0.01),and the levels of the SOD in rats of C group at different time after injure were significantly lower than before injure(P < 0.01),the severe traumatic MODS may involved in the inflammation,oxidative stress,and blood coagulation dysfunction,etc.3)The levels of the sTREM-1,sICAM-1,MDA,D-2G,and FDP in rats of TMP group or INS group at different time after injure were significantly lower than the C group(P < 0.01),and the levels of the SOD in TMP group or INS group at different time after injure were significantly higher than C group(P < 0.01),both treatment of TMP and insulin had effects of anti-inflammatory,anti-oxidative stress,anti-blood coagulation dysfunction caused by severe traumatic MODS.4)The levels of the sTREM-1,sICAM-1,MDA,D-2G,and FDP in rats of TMP+ INS group at different time after injure were significantly lower than the TMP or INS group(P < 0.01),and the levels of the SOD in TMP + INS group at different time after injure were significantly higher than TMP group or INS group(P < 0.01),treatment of TMP combined with insulin had better effects of anti-inflammatory,anti-oxidative stress,anti-blood coagulation dysfunction caused by severe traumatic MODS than the treatment of TMP or insulin alone.5)There was no statistical difference of rats mortality in 7 days among the TMP group,INS group,TMP + INS group and C group,although the rats mortality in 7days of TMP group,INS group,TMP + INS group were higher than C group.Neither combination nor single treatment by TMP and insulin can reduce markedly themortality of severe traumatic MODS.Conclusion:1)Severe trauma MODS rats were involved in inflammation,oxidative stress and abnormal blood coagulation.2)Both the TMP and insulin may reduce the inflammation,oxidative stress and coagulation abnormality of the scalded MODS rats,TMP and insulin may be used as effective drugs for the prevention and treatment of severe traumaic MODS.3)The of combination of traditional Chinese and western therapy,treatment of TMP accompanied with insulin,may reduce the blood indicators of inflammation,oxidative stress and blood coagulation abnormalities in severe trauma MODS rats,which may be better than treatment of TMP or insulin alone.The effects of TMP accompanied with insulin on the prevention and treatment of severe traumaic MODS may be be superior to TMP or insulin alone.4)The mechanism of TMP and insulin for the prevention and treatment of severe traumaic MODS may be reducing inflammation,oxidative stress and blood coagulation abnormalities.5)TMP and insulin therapy may decrease the mortality of the severe burns MODS,but no statistical significance,hinting the treatment of severe trauma MODS need comprehensive cure including other measures.The number of rats studied was not enough may be the reason of no statistical significance of mortality decreased by TMP and insulin therapy.
