A Clinical Study Of The Value Of CT Perfusion Imaging In Diagnosis And Predicting Hemorrhagic Transformation Of Acute Ischemia Stroke

Objective: To evaluate the clinical value of CT perfusion imaging in diagnosis and predicting hemorrhagic transformation of acute ischemia stroke.Methods: Thirty patients with acute ischemic stroke were examined by CT non-contrast scan and CTP,the CTP images was processed by VPCT Neuro-STROKE on the image workstation(Syng.VIA),the blood perfusion parameters were measured and ischemic penumbra was analyzed.Combined with follow-up CT non-contrast scan data,the CT non-contrast scan and CTP images were retrospectively analysised.Results: Of the 30 patients,16 had a low-density change in the head CT non-contrast scan and the remaining 14 had no significant abnormality.CTP showed ischemic positive lesions in 30 patients,and showed lesions more than CT non-contrast image,with ischemic penumbra.All has ischemic penumbra by CTP,there were significant statistical difference;CTP showed that there were significant differences in rCBF,rCBV,MTT between ischemic penumbra and cerebral infarct area in 16 patients with focal lesion(P<0.05).The CTP of 14 cases with no lesions in CT non-contrast images showed that the difference of rCBF and MTT in ischemic penumbra was statistically significant(P<0.05),While the rCBV difference was not statistically significant for the contralateral control area(P>0.05).In 30 cases of ischemic penumbra,9 cases occurred HT,21 cases did not occur HT.The rCBF and rCBV values of the ischemic penumbra in the HT group were lower than those in the no-HT group(rCBF and rCBV),and the difference was statistically significant(P<0.05),However,the MTT values of the two groups were similar,the difference was not statistically significant(P>0.05).Conclusion: CTP is able to diagnose acute ischemic stroke early and has a high sensitivity to ischemic penumbra.At the same time,CTP has great value in judging whether a patient has undergone hemorrhagic conversion after treatment.
