Objective1.To study the cognitive impairment in patients with chronic heart failure(CHF).2.To further understand the social support and emotional characteristics of CHF patients with cognitive impairment.3.To investigate the effect of aerobic exercise combined with cognitive behavioral therapy on cognitive function damage in CHF patient.Method1.100 cases of CHF patients in Jiangsu Rongjun Hospital were selected as the study group,50 healthy community volunteers were selected as normal control group,using the Montreal cognitive assessment scale(MoCA)to assess the cognitive function of two groups of patients,the difference in cognitive function were compared between the two groups.2.The incidence of cognitive impairment of CHF patients was calculated base on 26 points of MoCA.Using social support rating scale(SSRS),Hamilton Anxiety Scale(HAMA),Hamilton Depression Scale(HAMD)of two groups of patients to evaluate social support,anxiety and depression,compared two groups of patients in the difference in social support,anxiety and depression.3.The existence of cognitive impairment in patients with CHF,according to their need to improve blood pressure,blood vessels,strong heart,diuresis,enlarge improve asthma and other conventional clinical heart failure treatment,were randomly divided into cognitive intervention group and intervention control group.MoCA evaluation was performed before and after the combined intervention of aerobic exercise combined with CBT,and the efficacy of the two groups was compared.Results1.The CHF group was compared to the normal control group,cognitive function assessment result was poor.Two groups in visual spatial and executive ability,attention,delayed recall,total score differences statistically highly significant(P < 0.01),the other no statistically significant differences in cognitive function.2.Of the 100 patients in CHF group,69 were MoCA scores <26 scores,and the incidence of cognitive impairment in CHF patients was 69%,and the CHF group was divided into cognitive normal group and cognitive impairment group.3.There were significant differences in social support,anxiety and depression between the cognitive impairment group and the cognitive normal group(P < 0.05).4.A comprehensive intervention of aerobic exercise combined with cognitive behavior after 3 months,the cognitive intervention group and intervention control group were statistically significant differences in visual spatial and executive ability,attention,delayed recall,the total score(P < 0.01);Before and after the comprehensive intervention,the differences of the cognitive intervention group in the visual space and executive ability,attention,delayed recall and total score were statistically significant(P <0.01).Conclusions1.69% of CHF patients had cognitive impairment,mainly in executive ability,attention,and delayed recall.2.CHF patients with low social support,combined anxiety and depression were more likely to have cognitive impairment.3.Aerobic exercise combined with cognitive behavioral therapy has some improvement in cognitive function of CHF patients.
