The Influence Of Different Host Factors To Clinical Features In Patients With Pulmonary Crytococcosis

Objective:75 patients diagnosed pulmonary crytococcosis from several hospitals of Tianjin beween 1999 to 2016 were collected to analysis the influence of different host factors to clinical features in patients with Cryptococcus infection by statistical comparison.Methods:The 75 patients were divided into two groups,with basic diseases group and without basic diseases group.The gender,age distribution,presence of pigeons exposure history,clinical manifestation,laboratory test,latex agglutination test results and chest imaging findings,etc were compared between the two groups.Results: PC patients were more and more than ever From 1999 to 2016.Two groups of 43 cases,32 cases respectively,in which men and women each had 42,33 cases,no gender distribution difference in both groups(P = 0.579).Their age were between 23 to 76 years old,the average age of 48.72±10.23,most of them between 41 to 50 years old,the average age of with basic disease 51.06 ±10.22,of without basic diseases 46.98±10.00,no statistical difference in onset age between two groups(P = 0.887).The patients with pigeon exposure history accounted for 25 cases(33.3%),no statistical difference between the two groups.the frequent clinical manifestation were cough,fever,sputum,chest tightness,chest pain,shortness of breath,hemoptysis,no statistical difference in both groups(P = 0.133).Fever was more in patients with basic diseases(P=0.010),well cough was more in others(P=0.043).White blood cells,neutrophils ratio,blood sedimentation,calcitonin original had no statistical difference in both groups(P value were 0.28,0.065,0.274,0.073),c-reactive protein in the two groups of patients with difference was statistically significant(P = 0.019).56 patients were tested by cryptococcal capsular polysaccharide of latex agglutination in serum,positive in 51 cases(91.1%),and negatie in 5 cases(8.9%),no statistical difference in both groups(P = 0.062).30 cases(40.0%)were involved on both sides of the lung,45 cases(60.0%)of unilateral lung,two groups of patients had statistical difference(P = 0.012).Single lesions were in 25 cases(33.3%),multiple 50 cases(66.7%),no statistical difference between the two groups(P = 0.141).38 cases(50.7%)were involved of the subpleural lesions,two groups were statistically significant(P =0.041),patients without the basis disease were more likely to involved in the subpleural lesions.26 cases(34.7%)were nodular mass type,were the most common imaging findings in 75 patients and in patients with basic diseases.The both groups statistically difference(P = 0.011).Infiltrating type change were in 25 cases(33.3%).Mixed lesions were the most common imaging findings in patients with basic diseases,22 cases(29.3%).Two groups of patients showed statistically significant(P = 0.018).Pleural effusion,halo sign,pull bronchiectasis,including air bronchogram,hollow imaging signs between the two groups had no statistical difference(P >
