The Application Of Clinical Nursing Path In The Endoscopic Variceal Ligation

Objective:(1)According to the Clinical nursing path table which American Association of southeast surgical nursing had drawed up,combined with the illness and treatment characteristics of cirrhotic patients with esophageal varices ligation,we developed a path table for cirrhosis esophageal varices ligation,and analysed the feasibility of nursing pathway which used in patients with cirrhosis and esophageal varices ligation.(2)Applying the nursing pathway table to the patients with liver cirrhosis and esophageal varices ligation,following the path table of contents for targeted and systematic treatment and nursing care,we observed the effect on using clinical nursing pathway on patients with liver cirrhosis who were treated with endoscopic variceal ligation.Methods:(1)We introducted the clinical nursing path table of the southeast Surgical Nursing Association of the United States,Compared the two contents of the path,and put them into comprehensive analysis of the treatment,diet,nursing care and other aspects of contents of the path.According to the characteristics of patients with cirrhosis esophageal varices ligation,we developed the corresponding nursing path table,and took advice from nursing expert to modify the table,then selected a certain number of patients to pretest,analyzed the feasibility of nursing path in patients with cirrhosis esophageal varices ligation.(2)Ninety patients with liver cirrhosis treated with endoscopic variceal ligation who Met the inclusion criteria were randomly divided into control group and experiment group.The control group was given routine nursing which including admission guidance,observation,health education,diet nursing,medication nursing,etc.,while the experiment group was treated by developed nursing care path table in cirrhosis esophageal variceal ligation patients.By means of nurse training of clinical nursing path,according to the route table of content,the trained nurse targeted in the therapy and nursing step by step.Comparing the rebleeding rate,patients satisfaction,hospital stay,hospitalization expenses and the mastery of health education knowledge of the two groups,these results were analyzed and summarized,then we can obtain the effect of using clinical nursing pathway.Results:(1)We carried out a preliminary test on the clinical nursing pathway table of cirrhotic patients with esophageal varices ligation.it is concluded that esophageal varices ligation of clinical nursing path table can reflect the disease nursing features of this disease,the path table in the treatment and nursing measures can lead to a more significant effect on the patients,so it is more reasonable and worth for further promotion.(2)The rebleeding rate in experiment group is 4.4%,it is lower than the rebleeding rate in control group which is 17.8%,the difference is significant;The hospital stay in experiment group is 8.2±2.7,and hospitalization expenses in experiment group is 14922.5±4424.8,both are lower than the control group,the difference is significant;while patients satisfaction in experiment group is 97.8%,obviously higher than the control group which is 82.2%,the difference is significant;and the score of knowledge test in experiment group is 25.2±2.0,higher than the control group which is 22.4±2.1,the difference is significant.Conclusion:(1)Nursing path table of cirrhosis esophageal varices ligation is reasonable,and nursing path in patients with cirrhosis esophageal varices ligation is feasible.(2)Clinical nursing pathway used in the Endoscopic Variceal Ligation patients can significantly reduce the rebleeding incidence of patients,shorten the hospitalization time,decrease the average hospitalization cost,and reduce health care spending;It can also strengthen the patients' compliance to the therapy and nursing work,actively cooperate with the treatment and nursing work,and improve the satisfaction of patients,so it has a positive effect on the treatment of these patients.
