Objective To analyze the changes of the platelet parameters in 927 infants with lower respiratory infection,and to find out the relationship with the degree of inflammation.And to provide reference guide the clinical diagnosis and treatment.Method In this study,927 hospitalized infants with acute lower respiratory infection were taken into account from January 2012 to July 2016 in The First Affiliated Hosptial of Fujian Medical University.All the infants were grouped respectively by White Blood Cells Counts(WBC),Neutrophil Counts(N),C-reactive protein(CRP),Procalcitonin(PCT)and Interleukin-6(IL-6).These infants were divided into Group Fever and Group Nonfever.And 570 infants who had a fever were divided into 5 groups.According to the levels of WBC,these infants were divided into Group WBC-A(WBC?10.0×109/L),Group WBC-B(WBC 10.0×109/L ~ ?15.0×109/L),Group WBC-C(WBC >15.0×109/L).According to the levels of N,these infants were divided into Group N-A(Neutrophil count wasn't decreased),Group N-B(Neutrophil count was decreased).According to the levels of CRP,these infants were divided into Group CRP-A(CRP?10.0mg/L),Group CRP-B(CRP 10.0mg/L~?40.0mg/L),Group CRP-C(CRP>40.0mg/L).According to the levels of PCT,these infants were divided into Group PCT-A(PCT?0.05ng/ml),Group PCT-B(PCT 0.05pg/ml~?0.5pg/ml),Group PCT-C(PCT>0.5ng/ml).According to the levels of IL-6,these infants were divided into Group IL6-A(IL-6?7.0pg/ml),Group IL6-B(IL-6>7.0pg/ml).And we had compared the platelet count(PLT),platelet distribution width(PDW),mean platelet volume(MPV)and plateletocrit(PCT)in each group.We finally analyzed the platelet parameters in these infants.The SPSS 22.0 was used for statistic analysis.Result In this study,927 hospitalized infants with acute lower respiratory infection were collected.In Group Fever,PLT and PCT in 3~4 day and 5~6 day lower than they were in Group Nonfever.There were significant differences between these groups in these groups.In Group WBC-A,WBC-B and WBC-C,PLT(×109/L)were 316.0±116.9,387.8±140.4 and 419.4±154.6(F=48.155,P < 0.01),PCT(%)were 0.322±0.111,0.393±0.140 and 0.427±0.144(F=52.414,P<0.01).But there were no significant differences between these three groups in MPV and PDW(F were 1.092 and 1.306 respectively,P>0.05).In Group N-A and N-B,PLT(×109/L)were 367.6±139.3 and 266.3±92.7(t=9.255,P<0.05),and MPV(fl)were 10.23±0.92 and 10.53±0.95(t=2.904,P<0.05),and PDW(fl)were 11.82±1.97 and 12.38±1.93(t=2.576,P<0.05),and PCT(%)were 0.372±0.135 and 0.278±0.094(t=8.616,P<0.05).In Group CRP-A,CRP-B and CRP-C,there were no significant differences between these three groups in PLT,MPV and PCT(P>0.05).In Group PCT-A,PCT-B and PCT-C,PLT(×109/L)were 378.8±140.9,359.9±140.1 and 304.1±107.2(F=12.115,P < 0.01),PCT(%)were 0.386±0.132,0.364±0.138 and 0.314±0.107(F=12.199,P<0.01).But there were no significant differences between these three groups in MPV and PDW(F were 0.475 and 0.827 respectively,P > 0.05).In Group IL6-A and IL6-B,PLT(×109/L)were 374.9±141.7 and 349.5±135.7(t=2.588,P<0.05),PCT(%)were 0.379±0.136 and 0.356±0.133(t=2.416,P<0.05).But there were no significant differences between these three groups in MPV and PDW(t were 0.537 and 0.630 respectively,P>0.05).Conclusion The changes of the platelet parameters in infants with lower respiratory infection can be the same as the changes of WBC,N,PCT and IL-6.The platelet parameters can diagnose Systemic Inflammatory Response Syndrome(SIRS)early in infants with lower respiratory infection,and they can be more accurate and sensitive when combine with other inflammatory biomarkers.The WBC,N,PCT and IL-6 can't be replaced completely by the platelet parameters in determining infants with acute lower respiratory tract who finally deteriorated into SIRS,but blood routine may be more convenient,fast and reliabled in dynamic monitoring.It can be valuable in diagnosis and treatment of infants with acute lower respiratory infection,and it can be worthy of clinical popularization.
