Objective Traditional thoracic surgery is usually traumatic,minimally invasive thoracoscopic surgery can be more effective to improve this problem.Thoracoscopic surgery is developing rapidly,from the initial thoracoscopic assisted small incision surgery to four-hole thoracoscopic surgery,and then gradually over to three holes,double hole and even now single-hole thoracoscopic surgery,not only to meet the people to reduce the pursuit of surgical scars,but also reduce the intraoperative injury and the occurrence of various types of postoperative complications.However,the difficulty of single-hole thoracoscopic surgery is very high,the incommodious operation space,the limited field of exposure,and the risk of damage to the blood vessel when separated is cleared.We designed a new type of electric hook to reduce the single-surgery difficulty of resection.Methods In this study,a new type of unipolar electric hook was designed,which consisted of three parts: electric hook head,electric hook probe and electric hook handle.Electric hook head for the metal copper texture of the "L" shape,the external increase with the match of the high-density ceramic protective cap which for the removable,the appearance of the coating with oxide layer;electric hook head connected with the handle.The handle can control the electric shaft of the hook;the handle can control the head and the it`s cover closed,the front of the handle has to adjust the hook head to turn the universal shaft,the rear end of the electrode terminal connection,connecting one end of the electrode.And the other end electrode of the electrode plate affixed to the patient constitutes a current loop.We underwent single-hole thoracoscopic lobectomy under the general anesthesia for three miniature pigs(weighing 25 to 28 kg).Through the use of intraoperative views given improvements,and then according to the views of the surgeon to improve the device.And then use the improved device for 30 cases of clinical single-hole thoracoscopic lobectomy,the use of new electric hook used in the operation time(min),intraoperative(D),postoperative complications(%),and postoperative chest tube retention time(D),etc.,and compared with 30 cases of traditional thoracoscopic electric hooks with single-hole thoracoscopic lobectomy(surgery were the same person).Results 1.Animal experiments confirm the safety and practicability of the new electric hook;2.The average time of operation was 193.2 ± 17.9 min using conventional electric hook and single lobectomy.Compared with the new electric hook,the operation time was shortened by 180.9 ± 19.2 min,p <0.05.Conclusions The new type of unipolar electric hook designed in this study can shorten the operation time and reduce the difficulty of this operation in the single-hole thoracoscopic lobectomy,and shorten the corresponding learning curve,which can be more beneficial to the development of the minimally invasive surgery.
