Meta Analysis Of The Randomized Controlled Trials On Yindan Xinnaotong Soft Capsule In Treatment Of Acute Cerebral Infarction

Objective:To evaluate the effectiveness and safety of Yindan Xinnaotong Soft Capsule for acute cerebral infarction by systematic review,which provides strong evidence for clinical practice.Methods:Randomized controlled trials(RCT)on Yindan Xinnaotong Soft Capsule in treatment of acute cerebral infarction were retrieved from electronic databases including Cochrane Database,Medline(OVID),Embase(OVID),Pubmed,Chinese Biomedical Database,CNKI,VIP,Wan fang Database.For each database retrieval time from January2002 to May 2016.The Jadad scale was used to evaluate the quality of the literature.Two reviewers independently sereened the eligible studies with confirmation of cross-cheek.Different opinions were consulted by the third Party.Meta analysis was done by Revman 5.3.Results:Seventeen original studies met inclusion eriteria,included 1494 patients.There were729 patients in treatment group and 665 patients in control group.Meta analysis showed that the total effective rate of Yindan Xinnaotong Soft Capsule in adjunctive treatment of acute cerebral infarction was higher than that of conventional treatment[RR=1.19,95%CI(1.12,1.25),P<0.00001].National Institute of Health Stroke Scale(NIHSS)score decreased was more than that of conventional treatment[MD=-1.73,95%CI(-2.53,-0.92),P<0.0001].Barthel Index(BI)score was higher[MD=7.93,95%CI(4.32,11.54),P<0.0001],while low density lipoprotein[MD=-0.59,95%CI(-0.73,-0.46),P<0.00001] and fibrinogen [MD=-0.92,95%CI(-1.40,-0.45),P=0.0001] were lower.Incidence of adverse reactions are lower than in conventional treatment group,but with no significant statistical difference between groups[RR=0.65,95%CI(0.27,1.57),P=0.34].Conclusion:Yindan Xinnaotong Soft Capsule in treatment of acute cerebral infarction could achieve a better result in clinical curative effect and security.At the same time it can reduce the blood viscosity and blood lipid.However,due to the overall quality of the literature included in this study is not high,and some of the statistical indicators of statistical heterogeneity is high,can only provide low-strength evidence for the clinical.The results still needs more rigorous design,high quality randomized controlled triasl to verify.
