Neuroendocrine Changes Have The Influence On Parkinson Disease With Dementia

Objective:The tset was detected plasma thyroid stimulating hormone(TSH),free triiodothyronine(FT3),free thyroxine(FT4),adrenocorticotropic hormone(ACTH),cortisol,glycosylated hemoglobin(HbAlc)to investigate the changes of neuroendocrine in Parkinson's disease without dementia and Parkinson disease with dementia,explore the potential relationship between neuroendocrine and cognitive impairment in Parkinson's disease and provide evidence for early prevention of cognitive impairment in Parkinson's disease.Methods:The changes of neuroendocrine were studied in 42 patients with Parkinson's disease without dementia,21 patients with Parkinson disease with dementia and 20 controlers.The general data of patients were collected,such as gender,age,length of schooling,course of disease and Hoehn-Yahr(H-Y)scale.The mini-mental state examination(MMSE)and activity of daily living(ADL)were did by two professional neuropsychological assessment.Parkinsion disease with dementia did Neuropsychiatric Inventory(NPI).The morning fasting venous blood of all subjects were obtained to detect plasma thyroid stimulating hormone(TSH),free triiodothyronine(FT3),free thyroxine(FT4),adrenocorticotropic hormone(ACTH),cortisol,glycosylated hemoglobin(HbAlc)with automatic biochemical detector.Statistical analysis was run using SPSS21.0 Chinese version software.The measurement data was indicated the mean±standard deviation.The comparison of two measurement data was made by independent-samples T test.PDD,n-PDD and control group were compared with One-Way ANOVA.multiple comparison of sample number,using LSD-t test.P<0.05 believes that the difference was statistically significant.Results:There was no significant difference in the age and education years of the PDD group and n-PDD group(P>0.05),the difference of the course of disease and H-Y score were statistically significant(P<0.05).The PDD group had a longer course of disease,and the H-Y score was higher.The level of TSH in PDD group,n-PDD group and control group were: 2.79±1.11,2.74±1.03,2.04±0.86.The level of FT3 in PDD group,n-PDD group and control group were:.28±0.77,4.41±0.69,4.86±0.96.The level of FT4 in PDD group,n-PDD group and control group were:14.99±2.35,14.77±2.65,13.22±2.19.Compared with the control group,the difference of the level of TSH,FT3 and FT4 in PDD group and n-PDD group was statistically significant(P<0.05).The difference of the above indicators between PDD group and n-PDD group was not statistically significant(P>0.05).The level of TSH and FT4 in Parkinson disease without dementia and Parkinson disease with dementia were higher than those in control group,while the level of FT3 in them were lower than those in control group.The level of ACTH in PDD group,n-PDD group and control group were:7.15±2.75,7.96±2.61,9.70±3.65.The level of cortisol in PDD group,n-PDD group and control group were:431.87±170.63,351.21±133.90,330.02±106.76.Compared with the control group,the difference of the ACTH in PDD group and n-PDD group was statistically significant(P<0.05).The difference of the ACTH between PDD group and n-PDD group was not statistically significant(P>0.05)..Compared with the PDD group,the difference of the cortisol in control group and n-PDD group was statistically significant(P<0.05).The difference of the cortisol between control group and n-PDD group was not statistically significant(P>0.05).The level of ACTH in Parkinson disease without dementia andParkinson disease with dementia were lower than those in control group.The level of cortisol in Parkinson disease with dementia was.higher than that in PDD group and n-PDD group.The level of HbAlc in PDD group,n-PDD group and control group were:6.01±0.59,5.91±0.84,5.41±0.42.Compared with the control group,the difference of the level of HbAlc in PDD group and n-PDD group was statistically significant(P<0.05).The difference of the level of HbAlc between PDD group and n-PDD was not statistically significant(P>0.05).The level of HbAlc in Parkinson disease without dementia and Parkinson disease with dementia were higher than those in control group.Conclusion:Compared with the control group,the level of TSH,FT4 and HbAlc in Parkinson disease without dementia and Parkinson disease with dementia were higher,while the level of ACTH,FT3 in them were lower.The level of cortisol in Parkinson disease with dementia was.higher.The level of TSH,FT4,FT3,HbAlc and ACTH may have no effect on Parkinson's disease with dementia.The elevated blood cortisol levels may have an impact on Parkinson's disease with dementia.Neuroendocrine changes may provide evidence for the prevention of Parkinson's disease with dementia.
