Background: Cardiovascular disease is the most common cause of deaths worldwide.In recent years,with the improvement of people's living standards and change of eating habits,lack of exercise,etc.,the incidence and mortality of CVD increased in China.Atherosclerosis(AS)was a chronic inflammatory disease,studies have indicated that it is a risk factor for CVD.Increased arterial stiffness is one of the manifestations of atherosclerosis,and is an independent predictor of CVD outcomes.Although the measurement of pulse wave velocity(PWV)is a widely accepted noninvasive method to assess arterial stiffness,its accuracy is affected by changes in blood pressure.Cardio-ankle vascular index(CAVI),which is calculated based on the stiffness parameter thus obtained,is theoretically independent of changes in blood pressure.With this distinct advantage,CAVI has been widely applied clinically to assess arterial stiffness in subjects with known cardiovascular diseases including those with diagnosed atherosclerosis.Fatty liver(FL),characterized by accumulation of fat in the liver,is a chronic liver disease with rapidly increasing incidence that was proposed as the hepaticmanifestation of metabolic derangement of the body.FL is closely linked with metabolic syndrome(MS),and is highly associated with obesity,type 2 diabetes mellitus,dyslipidemia.Studies have also shown that FL is an active contributor to the pathogenesis of atherosclerosis and is not just a marker for CVD.Objective: To investigate the possible link between cardio-ankle vascular index and fatty liver of physical examination population.Method: 3218 subjects who underwent the health check-ups in the department of physical examination of The First Affiliated Hospital of Chongqing Medical University from January 2015 to December 2015 were enrolled and screened in this study.Based on their presence of fatty liver or not(evaluated by ultrasonography),these subjects were divided into two groups,the control group(without fatty liver,n=2298)and fatty liver group(n=920).The results of the two groups were analyzed by the statistical analysis.CAVI and the incidence of arteriosclerosis(CAVI ? 9 m/s as elevated arteriosclerosis)were compared between the two groups.Logistic regression analysis was performed to analyze the risk factors of fatty liver.Results:1.There was significant difference of the proportion of male to female in both control group and fatty liver group,and the proportion of male was higher than the female(P<0.05).2.The fatty liver group had significant higher CAVI than the control group(8.00±1.14 vs7.80±1.07,P<0.05)3.The incidence of arteriosclerosis in the fatty liver group was significantly higher than that in the control group(P<0.05)4.To carry out the multivariate logistic regressions with the gender,BMI,SBP,DBP,LDL-C,HDL-C,TG,FPG and CAVI as the independent variate,and the presence of fatty liver or not as the dependent variate,the results showed that higher BMI,TG,FPG and CAVI were the risk of fatty liver,the association between CAVI and fatty liver remained significant after adjustment for confounders.Conclusion: The association between CAVI and fatty liver are significant.So,early detection of atherosclerosis with fatty liver in physical examination population has potential benefits in prevention of cardiovascular diseases.
