| Background and objective: As one of the most common complications of acute cerebral infarction,stroke in progression(SIP)is the exacerbation of neurological deficit within 6 hours to a week in patients with cerebral infarction.For SIP occurred within 48h-72 h,it is defined as early neurological deterioration(END).Since many factors may influence the risks of END,there is no definite conclusion.In this regard,we collected clinical data and analyzed the factors influencing END in order to provide basics for clinical diagnosis and treatment,thus improving the prognosis of patients.Method : In this paper,systematic retrospective analysis was firstly conducted on 389 patients with acute cerebral infarction.Then,the patient’s clinical data,clinical manifestations,the clinical features and laboratory results were collected.Last,the differences between the two groups were analyzed by spss24.0 to find out the risk factors of END.Results:(1)Among the 389 patients with acute cerebral infarction,66 patients were met the diagnostic criteria of END,and the corresponding incidence was 16.97%.(2)General information :There was no statistically significant difference between the END group and non-END group in the general data,showing no correlation with END.(3)Clinical manifestations :The temperature of END group and non-END group were 36.768±0.572℃,36.526±0.641℃,the difference between the two groups was statistically significant(P=0.003).The NIHSS score of END group(9.76±5.969)were higher than those of non-END group(5.97±4.799)on admission,and the difference between the two groups was statistically significant(P<0.001).Moreover,there was no significant difference in systolic and diastolic blood pressure between the two groups.(4)Laboratory test results: Elevated blood glucose level and high Fib are independent risk factors for the occurrence of END.(5)Imaging findings: END group(47.676±47.873cm3)in infarct volume than non-END group(15.502±29.299 cm3)was significantly raised,the difference between them was statistically significant.(6)Multiple logistic regression analysis :After excluding confoun ding factors,the multivariate logistic regression analysis,showed that,high glucose(P=0.011,OR=1.084,95%CI[1.018-1.153]),high NIHSS sc ore(P<0.001,OR=1.109,95%CI[1.049-1.171]),the elevated temperature on admission(P=0.011,OR=2.532,95%CI[1.233-5.200]),high level of Fib(P=0.033,OR=1.353,95%CI[1.024-1.787]),large infarct volume(P<0.001,OR=1.015,95%CI[1.008-1.023])are five independent risk factor s for the occurrence of END.Conclusion: END is one of the most common complications in patients with cerebral infarction,and there are many risk factors influencing END.High fasting plasma glucose level,high level of Fib,high NIHSS score,large infarct volume and elevated temperature on admission are found to be independent risk factors for the occurrence of END. |
