Objective To evaluate the clinical analgesic efficacy of dexmedetomidine combined with sufentanil on uterine contraction pain after transabdominal hysteromyomectomy.Methods 60 cases of patients applied with ASA I or II general anesthesia in hysteromyomectomy,they are divided into two groups according to randomized double-blind control?n=30?: Group S received 0.04 ug·kg-1·h-1 of sufentanil,Group Y received 0.025 ug·kg-1·h-1of sufentanil combined with 0.03 ug·kg-1·h-1of dexmedetomidine,both groups received compound use of 8 mg Ondansetron and 2.5mg Inapsine,which are diluted into 100 ml with normal saline,with background infusion at 2 ml·h-1,PCA dose of 2 ml and a 10 min lockout interval.PCA pump was immediately connected after the operation to record the BCS comfort scores,VAS scores,Ramsay sedation scores,effective pressing number within 48 h,dose of sufentanil,nausea and vomiting scores,MAP,HR,SPO2% at 4 h?T1?,8 h?T2?,16h?T3?,24 h?T4?and 48 h?T5?and satisfaction of patients within 48 h.Results The comparison on ordinary circumstances?age,weight,height?and operation condition?operation time and anesthesia time?is not statistically significant?P>0.05?,which aged from 24-52,BMI of 19.43 29.69kg·m-2.As compared with Group S,MAP of Group Y at 4h declined,the difference was statistically significant?P <0.05?,at8 h,12h,16,24 h and 48 h was not statistically significant?P>0.05?.SPO2 value of both groups were greater than 98%(postoperative conventional low-flow oxygen2L·min-1),not statistically significant?P > 0.05?.Compared with Group S,the self-controlled effective pressing number declined,the dose of sufentanil declined,the possibility of nausea and vomiting rate declined at 4 h?T1?,8 h?T2?,16 h?T3?,24 h?T4?and 48 h?T5?,the difference was statistically significant?P <0.05?,no patient was applied with remedial drugs.Compared with Group S,the VAS scores of Group Y were lower,and BCS scores of Group Y were higher at 4 h?T1?,8 h?T2?,16 h?T3?,24 h?T4?and 48h?T5?,the difference was statistically significant?P <0.05?.The Ramsay scores of Group Y at 4 h?T1?,8 h?T2?,16 h?T3?and 48 h?T5?increased,the difference was statistically significant?P <0.05?,there was no significant differerce at 24 h time points?P>0.05??Conclusion Based on the experimental study,the sedation formula of 0.03ug·kg-1·h-1dexmedetomidine and 0.025 ug·kg-1·h-1sufentanil not only significantly reduced the degree of uterine contraction pain,made hemodynamic steady with obvious effect of sedation,but also reduced the dose of sufentanil,which could reduce the possibility of adverse reactions,improve the comfort and satisfaction of patients,as well as improved the postoperative analgesia effect on patients after applied with transabdominal hysteromyomectomy.
