Objectives: Through the investigation and statistics of the clinical data of patients with uterine sarcoma,and to study the prognostic factors of uterine sarcoma,in order to understand the related factors of uterine sarcoma incidence and pathological type,and prognosis of uterine sarcoma,and prevention of uterine sarcoma and provide effective clinical evidence,improve the survival time of patients.Methods: A retrospective analysis of the affiliated hospital of Qingdao university of 36 cases of uterine sarcoma treated over a period of nearly 10 years of age,starting symptoms,surgery way,whether chemotherapy and prognosis,and postoperative follow-up of 1 year,5 years were analyzed.Results: The study of 36 patients with uterine sarcoma,the youngest is 24 years old,age at most 82,average age 47.53 ±12.37 years,according to the international union of gynecology and obstetrics(FIGO,2009)standard: staging stage I 22 cases,8 phase II,phase III in 2 cases,IV 4 cases.According to the sarcoma of uterus histological classification standard,18 patients with LMS,7 patients with ESS,the AS patients in 2 cases,7 patients with CS,2 patients with other pathological types.Calculate the median age of 46.5 years,with its bounded,data can be divided into two groups,the line of chi-square give different pathological type of uterine sarcoma there are differences in the onset age(P = 0.027),namely the AS,ESS patients age small,CS onset age patients with the highest.1.The first symptom of uterine sarcoma clinical symptoms lack specificity and realizable for vaginal bleeding(47.2%,17/36),abdominal mass(25%,9/36),abdominal pain(5.6%,2/36,including 1 case of merge abdomen bag piece),vaginal drainage(2.8%,1/36),such as the study group 36 cases,11 cases of preoperative misdiagnosed as uterine fibroids,8 cases of preoperative diagnostic curettage,1 case of preoperative diagnostic abdominal puncture,14 cases of intraoperative frozen pathology confirmed for uterine sarcoma.2.Maternal times of uterine sarcoma develops in women over 40 to 60 years old,in this study,the group of 36 cases with a history of multiple births,15 cases(2 times and above),14 cases of single birth history,barren history and 4 cases,3 cases of reproductive history is unknown.Chi-square test,it is concluded that whether it is can not influence the incidence of multiple births,and presence of reproductive history army.3.The operation method The study of 36 cases of uterine sarcoma patients,1 case for preoperative biopsy consider abandoning treatment for uterine leiomyosarcoma,more than 35 cases were performed surgery.Among them,there are total 9 cases patients hysterectomy bilateral adnexectomy Pelvic lymph nodes were performed Total resection of the greater omentum resection,6 underwent hysterectomy bilateral adnexectomy Pelvic lymph node cleaning,16 patients whole uterus bilateral adnexectomy,2 patients with family members to keep the uterus uterus tumor resection,the other 2 cases for the im considering transfer lines the uterus bilateral adnexectomy Pelvic lymphadenectomy greater omentum resection Part of the bowel resection.4.Survival and prognostic analysis of 36 cases of 21 cases of death,in which 11 cases died of uterine sarcoma has a relapse,the recurrence rate was 30.6%(11/36),recurrence of an average of 2.3 years,1 year survival rate 65.63%(21/32),the 5-year survival rate was 43.75%(14/32).Each different stages and histopathological types of uterine sarcoma clinical followup data were separately K-M method,the Log-Rank survival curve method of single factor analysis,it is concluded that clinical stage,pathological type,respectively the independent factors affecting the prognosis of uterine sarcoma.Conclusion: The vaginal bleeding in different age and the first symptom affected the pathological type of uterine sarcoma.The age of onset of AS and ESS was smaller,the age of onset of CS was the highest,and the rate of vaginal bleeding in LMS patients was small.The survival rate of uterine sarcoma is closely related to the pathological type and pathological staging of uterine sarcoma.The pathological type cancerization degree is higher,the higher the pathological stage,the survival rate is smaller.Chemotherapy has a good effect on survival rate of patients with uterine sarcoma.The prognosis of uterine sarcoma and pathological type,clinical stage and treatment are closely related.Clinical stage,pathological type,whether systemic chemotherapy for the independent factors affecting the prognosis of uterine sarcoma.
