| Objective: To investigate the relationship between plasma placental growth factor(PLGF)and type 2 diabetes mellitus and its macrovascular complications. Methods: 58 patients with type 2 diabetes mellitus were divided into two groups according to the presence of diabetic macrovascular complications:group T2DM(28 cases without diabetic macrovascular complications)and group T2DM+MVC(30 cases with diabetic macrovascular complications).According macrovascular inclusion criteria, select the same period 30 patients without diabetes into MVC group.24 healthy individuals were served as controls(group NGT).Interrogate all subjects of the past medical history, get all subjects of the height, body weight,BMI,fasting blood glucose(FPG),total cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),glycosylated hemoglobin(Hb A1c),fasting C peptide, hypersensitive C-reactive protein(hs-CRP)and PLGF.Serum PLGF was detected by ELISA.Results:(1)The levels of serum PLGF in the groups of T2 DM,MVC and T2 DM + MVC(13.99±6.62,17.67±7.44,41.37±8.65)pg/ml were higher than those in NGT group(9.67 ± 6.59)pg/ml(P <0.01).The levels of serum PLGF in T2 DM + MVC group were significantly higher than those in T2 DM and MVC groups(P <0.01).There was no significant difference in serum PLGF between T2 DM and MVC groups(P = 0.06).(2)Serum PLGF was correlated positively with FPG,TC,TG,Hb A1 c,hs-CRP(r = 0.486,0.300,0.217,0.427,0.519,P<0.05)in all subjects, and with FPG,Hb A1 c,hs-CRP(r=0.661,0.625,0.352,P<0.05)in patients with atherosclerosis. The level of serum PLGF in type 2 diabetes mellitus was positively correlated with hs-CRP,TC and LDL-C(r = 0.444,0.390,0.278,P <0.05),and negatively correlated with fasting C peptide(r =-0.332,P <0.05).(3)Logistic regression analysis showed that PLGF,LDL-C and hs-CRP were the risk factors of atherosclerosis, and HDL-C was the protective factor.Conclusion:(1)In patients with type 2 diabetes and atherosclerosis alone, serum levels of PLGF were higher than those in normal subjects.Serum levels of PLGF were further elevated in patients with type 2 diabetes mellitus complicated with macrovascular complications.Assosiating with the level of PLGF positively correlated with hs-CRP,suggesting that PLGF may promote the occurrence and development of diabetes mellitus and its macrovascular complications by participating in inflammatory reaction in the pathogenesis of type 2 diabetes and atherosclerosis. Thus,PLGF may be an early predictor of macrovascular complications of type 2 diabetes, and also provide a new possible entry point for early intervention.(2)PLGF was positively correlated with fasting blood glucose,a variety of lipid and hs-CRP.According of controlling hypoglycemia, lipid, weight and other methods, may reduce the level of serum PLGF,and delay the occurrence of diabetes and its macrovascular complications, and reduce the incidence of life-threatening cardiovascular and cerebrovascular events. |
