Objective Three-dimensional Speckle Tracking Imaging(3D-STI)is a novel software mainly used to evaluate the structural and functionalchanges of heart ventricle now,however,the clinical application in atrium assessment is less.Sick Sinus Syndrome(SSS)is one of the common heart diseases in clinic,which easily accompanied with various arrhythmias.In this study,we used 3D-STI to assess the remodeling extent of atrial structure by measuring the volume of the atrium,meanwhile,assessing whether the patients have atrial dyssynchrony by measuring the globle atrial muscle strain rate,finally,evaluating the degree of atrial electrical remodeling.Methods Choosing the patients who were diagnosed with SSS for the first timeduring2014.12~2016.10 in the affliated hospital of Qingdao University.The patients were divided into two groups according to whether they were complicated with bradycardia-tachycardia syndrome.The first group(SSS1 group)was a total of 25 patients with bradycardia-tachycardia syndrome,while the second group(SSS2 group)was a number of 25 patients without bradycardia-tachycardia syndrome,25 normal subjects were taken as the control group.Left atrial maximal volume(LAVmax),left atrial minimum volume(LAVmin)and left atrial systolic volume(LAVp)were measured and recorded.The left atrial volume index(LAVI),left atrial stroke volume(LASV),left atrial passive ejection fraction(LAPEF)and left atrial active ejection fraction(LAAEF)were calculated from the above data,while measuring right atrial volume(RAV)and calculating the right atrial volume index(RAVI).The global longitudinal peak strain(GLPS),global radial peak strain(GRPS),global circumferential peak strain(GCPS),global area peak strain(GAPS)were obtained by 3D-STI technique.All data were recorded and processed using SPSS 17.0 statistical software.The above indicators were used to evaluate the degree of atrial synchronization loss and structural remodeling,and to compare whether the degree of reconstruction was associated with patients with bradycardia-tachycardia syndrome.Ultimately,it is of reference for 3D-STI value in evaluating the atrial electrical and structural remodeling in SSS patients according to the results.Results1.Compared with the control group,LAVp and LAVI were increased in both SSS1 and SSS2 groups,and LASV,LAPEF and LAAEF were decreased(P<0.05).But there was no significant difference between SSS1 group and SSS2 group.2.Compared with the control group,RAV and RAVI were increased in the SSS1 group and SSS2 group(P<0.05),there was no significant difference between the SSS1 group and the SSS2 group.3.Comparisonin three groups(control,SSS1,SSS2),GLPS,GRPS,GCPS and GAPSshowed an increasing trend,SSS1 group is the lowest among three groups(P<0.05).Conclusion1.Compared with the healthy people,SSS patients have atrial electrical remodeling and structural remodeling.2.The degree of electrical remodelingstructural remodeling in SSS patients with bradycardia-tachycardia syndrome is more serious than the patients without bradycardia-tachycardia syndrome.3.3D-STI can quantitatively assess the structure and function of atrial myocardium,whichcould fill the shortcomings of conventional echocardiography.3D-STI could evaluate the patients' heart function more timely and perfectly,which provide a reference for the early clinical intervention of SSS treatment.
