Objective: To investigate the diagnostic value of serum C-reaction protein to albumin ratio for spontaneous bacterial peritonitis in cirrhotic patients.Methods: A total of 110 patients with cirrhosis complicated with ascites were included in the study.They were divided into two groups: 35 cases of SBP group and 75 cases of non-SBP group.The peripheral blood leukocytes,percentage of neutrophils,serum C-reactive protein,albumin,serum C-reactive protein and albumin ratio were analyzed statistically.The ROC curve was used to compare the diagnostic value of different indexes in cirrhosis with spontaneous peritonitis.To find the best cut-off of C-reactive protein and albumin ratio in the diagnosis of liver cirrhosis complicated with spontaneous bacterial peritonitis.SPSS 22.0 was used for statistical analysis,P<0.05 indicated that the difference was statistically significant.Results: The median level of CRP/ALB in SBP group was 0.78(0.44,1.61),which was significantly higher than that in non-SBP group 0.09(0.05,0.16),the difference was statistically significant(P<0.05).The area under the ROC curve of CRP/ALB diagnostic SBP was 0.969,which was significantly different from the area under the curve of WBC and NEUT%(P<0.05),and there was no significant difference with the area under the curve of CRP(P=0.48>0.05).The cut-off value of CRP was 6.69mg/L,the sensitivity of CRP in diagnosing SBP was 88.60% and the specificity was 94.7%.When the cut-off value of CRP/ALB was 0.21,the sensitivity of CRP/ALB in diagnosing SBP was 94.30% and the specificity was 90.70%.Conclusion: Serum C-reactive protein to albumin ratio has a potential value in the diagnosis for spontaneous bacterial peritonitis in cirrhotic patients,its sensitivity is better than C-reactive protein.
