Objectives: Comparison of pathologic changes of cartilage endplate in different grades of cervical disc degeneration.Methods: The degree of degeneration of the human cervical disc was determined by the Pfirrmann grading method of intervertebral disc MRI.Selected from Ningxia Medical University General Hospital Spine Orthopedics from June 2016 to December 2016 due to cervical degenerative disease in patients with surgical treatment of cartilage endplates in 40 cases.Group A was grade I,group B was grade III,group C was grade III,group D was grade IV,group E was grade V,and group 8 was included in each group.All cases were routine X-ray,CT reconstruction and nuclear magnetic resonance imaging,surgical methods were anterior cervical discectomy and fusion(AC DF).The morphological changes of cartilage endplates were observed by HE staining and toluidine blue staining.The expression of Caspase-3 and type X collagen in cartil-age endplate cells was detected by immunohistochemistry,and the results were analyz-ed statistically.Results: 1.HE staining: A group of chondrocytes in the shape of chondrocytes si-milar to the spherical,extracellular matrix red dye,symmetry,the number of cells more cartilaginous lacunae are more.The number of cartilage cells in group B was decreased,the cartilage lacunae was reduced,and the fibrous tissue around the cells was cluttered.C group cartilage end plate chondrocytes shrink,cartilage lacunae was significantly reduced,part of the cell interstitial light stained,the matrix was fibrosis.D group cartilage end plate cartilage lacuna structure blurred,cytoplasm can see a large number of vacuoles,accompanied by the disappearance of nuclei,chondrocytes near the connective tissue can see the vitreous Degeneration.The number of chondrocytes in the chondrocytes of group E was the least,and the structure of cartilage lacunae was difficult to distinguish.The fissure was seen in the matrix,and the hyaline degeneration in the connective tissue was also more serious.2.Toluidine blue staining: In group A,the cartilage endplate staining was dark blue and purple,and the thickness was thick,the number of chondrocytes was more,and the extracellular matrix was also abundant.In group BE,the staining of cartilage endplate gradually fades,and the thickness gradually thinned,the number of chondrocytes and the extracellular matrix were gradually reduced.The density of chondrocytes in group B was the highest,and the color of chromatin was the highest in group C and D,and the density of chondrocytes in group E was the lowest.3.Caspase-3 immunohistochemistry: Caspase-3 expre-ssion was found in the cartilage endplates of each group and expressed in the cytoplasm.With the elevation of MRI degeneration of cervical discs,caspase-3 expression in cartilage endplate cells more and more deep,and the number of positive cells gradually increased.The positive rate of group A was 16.34 ± 2.62%,the positive rate of group B was 40.27 ± 3.26%,the positive rate of group C was 47.13 ± 5.69%,the positive rate of group D was 68.27 ± 4.58%,the positive rate of group E was 74.82 ± 3.53%.There was significant difference between the five groups(P <0.05).4.X-type collagen immunohistochemistry.No obvious positive staining was found in group A cartilage endplate.While in the B-E group of cartilage endplates were found to ha-ve X-type collagen expression,and expressed in the cytoplasm and hypertrophic chondrocytes around.With the increase of cervical disc degeneration,the expression of type X collagen in cartilage end plate cells was more and more deep,and the number o-f positive cells gradually increased.The positive rate of group A was 2.18 ± 0.9%,the positive rate of group B was 14.04 ± 1.24%,the positive rate of group C was 22.12 ± 2.54%,the positive rate of group D was 31.73 ± 3.34%,the positive rate of group E was 41.43 ± 2.38% There was significant difference between the five groups(P <0.05).Conclusions: 1.There was a difference in the pathological changes of cartilage e-ndplate in different grades of cervical disc degeneration.2.The expression of Caspase-3 and type X collagen in cartilage endplate cells was positively correlated with MRI grade of disc.3.Pfirrmann grade of intervertebral disc degeneration is consistent with cartilage endplate degeneration and can reflect the degree of cartilage endplate degeneration at the cellular level.
