Clinical Study On Phlebography's Value Of Lower Limbs For The Diagnosis Of Superficial Varix Of The Lower Limbs And Cockett Syndrome

Objective:I survey the prevalence of Cockett syndrome in patients suffering Superficial Varix of the lower limbs.I survey the veracity,sensitivity and missed diagnosis rate of Color Doppler Ultrasound by using Phlebography as the standard for diagnosing the Cockett syndrome and contraindication of LVSV.Besides,we compared the different results of operation rate,constitution of contraindications and incidence of lower extremity Deep Venous Thrombosis about the two different diagnostic methods,to evaluate the diagnosis and antidiastole value of Phlebography for lower limbs.Method: We collect 115 patients from January 1,2016 to December 31,2016,who had Superficial Varicose Veins of the lower limbs,investigate the prevalence of its Cockett syndrome by Phlebography.And I check the sensitivity and missed diagnosis rate of Color Doppler Ultrasound.Taking 193 patients,from October 1,2014 to October 31,2016,we Divided them into two groups,group A(Color Doppler Ultrasound only)with 59 patients,group B(Phlebography)with 134 patients.I check the operation rate,constitution of contraindications of the two groups' patients.All patients with traditional surgery,high ligation and stripping of varicose vein,accepted Routine dose of low molecular heparincalcium,adjusted by Parameters of coagulation function's daily results,were used postoperatively by skin injections.According to the follow-up results of the patients with deep vein thrombosis(DVT),I record the dates and analyze by statistics software,to analysised the two meathods' effection for DVT after surgery.For the 134 patients of group B,I check the veracity,sensitivity and missed diagnosis rate of Color Doppler Ultrasound by using Phlebography's results as the standard.Result:(1)The study found that there were 15 patients suffered with Cockett syndrome among the 115 patients,who was suffering Superficial Varicose Veins of the lower limbs at the same time,diagnosised by Phlebography.The prevalence of Cockettsyndrome is about 13.0%.The Color Doppler Ultrasound's results were 1.7%.The results of sensitivity and missed diagnosis rate of Color Doppler Ultrasound is :SE=13.3%,? =86.7% by using Phlebography as the standard for diagnosing the Cockett syndrome.(2)Recording the 134 patients who accepted Phlebography and Color Doppler Ultrasound's results for Superficial Varix of the lower limbs' different contraindication results.We got veracity of Color Doppler Ultrasound results was that : deep venous valve insufficiency =91.5%,DVT=100%,Cockett=29% by using Phlebography as the standard.(3)Recording the 193 patients,who accepted traditional surgery of venous varicose.We got 8 patients(3 men,5 women)suffered with DVT from the group A.The prevalence of DVT is about 15.3% of group A.Group B had 4(2 men and 2women)patients suffered with DVT postoperatively,it's about 5.1 %.Using continuous correction test,we got this result: ?2=4.23,P=0.04<0.05.That means the result reach statistical significance.(4)By checked the constitution of contraindications of the two groups' patients,I found that : deep venous valve insufficiency came first.the result is ?2=1.92,P>0.05.That means the result reach statistical significance.Conclusion:(1)The prevalence of Cockett syndrome of our department is about 13.0% in2016 by using Phlebography as the standard.The Color Doppler Ultrasound's sensitivity and missed diagnosis rate were higher for Cockett syndrome's diagnosis than Phlebography.(2)Phlebography had a high screening result for traditional surgery,led to a low prevalence of DVT.(3)Color Doppler Ultrasound cannot replace the position of Phlebography'value for diagnosis of Superficial Varicose Veins of the lower limbs.But it can be used as Superficial Varicose Veins' screening.(4)For the patients who had Superficial Varicose Veins of the lower limbs,we take Color Doppler Ultrasound or Phlebography at the right time is much more advantage.
