The Research Of Ultrasonic Examination In Carpal Tuunel Syndrome

Objective: With social progress and economic development,Median nerve injury become more and more because of the change of people work.Tunnel carpal is a bone fibrous structure made up of carpal and ligamenta carpi transversum in which to walk include flexor digitorum superficialis,Median nerve,flexor pollicis longus muscle tendon,deep flexor tendon.Median nerve walk between flexor pollicis longus muscle tendon and send back to dominate musculi abductor pollicis brevis,musculi flexor brevis,musculus opponens pollicis.The end branch of median nerve is digital nerves which dominate the skin of thumb,forefinger,middle finger and the radial side of ring finger.The main cause of CTS have great connection with the increase pressure in tunnel carpal.Whatever cause increased soft tissue in tunnel carpal or the volume decrease of tunnel carpal,the pressure in carpal tunnel increase.The compression of median nerve can cause the dysfunction of the innervation area,which indicate decreased sensation of the skin of thumb,forefinger,middle finger and the radial side of ring finger,thenar atrophy and opposition dysfunction.CTS is the most common type in peripheral nerve injury.The primary age of CTS is between 40 and 60,and the female morbidity is higher than the incidence of male.The reason caused increase pressure in tunnel carpal is a great many,for example synovial hyperplasia and fibrosis,synovial cyst,nerve fiber cyst,lipoma and hematoma of carpal tunnel,and some less reason as too low of flexor muscle of the abdomen and textural anomaly caused by traumatic or degenerative change.The change in tunnel carpal is throughout the cure process in the treatment of CTS.Peripheral nerve system which consist of nerve,ganglion,receptor,cynapse and motor nerve ending can report human body information.Becausethe different change of EMG can judge the damage location,the quantification of the severity,the damage properties,EMG have considerable effect on the pathogenesis of diseases,clinical diagnosis,prognosis of nerve injury.EMG is considered as the gold standard of peripheral nerve function,but EMG can not diagnosis nerve injury and prognosis when nerve injury within 3-4 weeks and nerve regeneration did not reach the effector.EMG can not do visual inspection in the damage location,the damage properties.MRI and ultrasonic can clearly show the soft tissue and median nerve in the carpal tunnel,reveal median nerve injury,display the cause of median nerve damage which provide an objective basis for diagnosis for CTS.Ultrasonic in the diagnosis of CTS is simple,quick and clear and can clearly show the soft tissue and median nerve in the carpal tunnel which have important guidance for the operation.Ultrasonic provides a new method for the diagnosis and research of nerve entrapment.Methods: 2016.06-2016-12 the 50 cases CTS who treated in our hospital and were eliminated primary raynaud`s syndrome and peripheral vascular occlusion and so on which can effect the experimental results are selected as observation group.50 healthy volunteer who were eliminated diseases of nervous and vascular system are selected as control group.The median nerve is examined by ultrasound and take the average of 3 measurements in the anteroposterior.And compare the difference between observation group and control group.Results: ultrasound clearly show median nerve arranged and organization shuctures.There are difference between observation group and control group(P<0.05).Conclusions: CTS is a disease which indicate decreased sensation of the skin of thumb,forefinger,middle finger and the radial side of ring finger,thenar atrophy and opposition dysfunction.Ultrasonic can clearly show the soft tissue and median nerve in the carpal tunnel which provide an objective basis for diagnosis for CTS.The experiment by wrist ultrasonography can simply and quickly show median nerve the damage location,the quantificationof the severity,the damage properties and median nerve arranged and organization shuctures.Ultrasonic play an important part in the diagnosis of CTS.
